Question: hi, I am 33 years old and just detected high blood sugar. Doctor prescribed ismat50/500 ( 1 tablet in morning) and eurepa 0.5( half tablet at nite). my fasting without medicine is 175( on average) i need to know since i am just 33 years old now, are there are side effect of these medicines in long run ? more over i would switch to Ayurveda medicine for the same instead of allopathy in long run. Please advice. I am working profession , but have introduced morning walk in my schedule and other physical exercise with no potato, sugar , rice in food

The tablets Istamet(Sitagliptin and metformin) and Eurepa(Repaglinide), both are hypoglycemic drugs helps you in controlling blood sugar levels. The average fasting blood sugar should be less than 110mg/dl to say well controlled. Good physical activity and low calorie small frequent meals also helps you in controlling the blood sugar levels. If not controlled after all this, discuss regarding slight increase of doses with your treating doctor. The known side effects of Istamet are Headache, Upper respiratory tract infection, GI disturbances, Rarely convulsions, ataxia, slurred speech, Emotional instability. Thanks for your response. After 4 days of course my fasting reduced to 118. My further query is : Do i have to take these drugs life long. Taking these drugs continuously of next 20-25 years... will lead to organ failure / impact on kidney. My above query is so, as i am only 33 years old now. Can it be controlled though diet ans exercise only. Do you foresee an any chance of stopping drugs.

Brief Answer: Diet and exercise can do for a while. Detailed Answer: Hi, Even 118mg/dl is slightly higher than good control(<110mg/l). Since you are only 4 days from starting the course, I would stop the antidiabetic drug. Encourage you to be strict with diet and exercise. I shall review your blood sugars after a month and manage you accordingly. You can discuss about his approach with your treating doctor. Strict Diabetic diet and exercise can help you for a while. But if the sugar levels rise again then drugs are mandatory. And coming to 2nd query, once you are to use the drugs, then they are usually given for rest of life unless you develop contraindications. There may be a slight changes in dosages or drugs, but continuation is necessary. You need not worry of complications, They are proven safe even in long term.
