Brief Answer:
Surgery would be a good option.

Detailed Answer:
Hi.
If its a comfirmed fissure then the protrusion would be a sentinel tag which is a protective mechanism of the body to protect the fissure area from coming in contact with anything. It also means that this has been happening to you repeatedly.
If the pain is too much then you should probably go in for surgery. It is a minor surgery wherein they will knock off a part of the sphincter and the pressure would get relieved. This along with preventing constipation would solve the issue.
There are other ways to decrease the pressure which you can try like GTN ointment or Diltiazem cream which can be applied locally and give you relief. But if you want a permanent solution I would suggest you opt for surgery.
Sitz baths will also give you relief in pain. You would have to collect warm water in a broad based vessel and sit in it for about 20 mins 2 to 3 times a day.
Take care and do let me know if there are any queries.
