                                            Question : Dear Doctor,
Yesterday, I had a sexual encounter with a new partner whereas she pulled out my penis to give me a handjob and in the mix she spit on my penis twice. I became uncomfortable and had her stop. I did not have any visible scars or cuts on my penis. Furthermore, I did not ejaculate. My concern is that some of her saliva may have seeped into my penis tip (uncircumcised) with the pre-cum. I was unable to see if her saliva had any blood in it given the lighting. When I arrived home a couple of hours later I washed my penis areas thoroughly with soap and water, urinated and ejaculated in hopes of getting out any possible contaminates.
I am concerned as I am usually a very cautious person. I was wondering what are the chances of getting HIV or other STDs from the described situation.
I sincerely appreciate your thoughts. Thank you!

                                    Dear Doctor,
Thank you for your earlier response indicating that saliva does not transmit HIV.
My specific concern is if the saliva potentially contained blood and if it got into my urethra (uncircumcised penis) or if my penis had any small cuts.
I know I sound crazy, but appreciate your response.

Detailed Answer:
Dear Mr XXXXXXX
Saliva is unlikely to contain blood unless there are oral bleeding lesion.
Even if the saliva comes in contact with Penis , the secretions are unlikely to enter the Urethra.

                                    Dear Doctor,
Thank you so much. Even if there were blood in the saliva, is it still unlikely that it would go into the urethra or be absorbed some other way? Please note that I did have precum but didn't ejaculate as I stopped the lady. Please note that I did not wash my penis for about two hours after this incident.
Sorry with the repeat questions. Many thanks. This is very untypical for me a and quite scary.
My Best Regards, God Bless You!
Dear Doctor -
One more question - Do you think I need PEP treatment?
Sincere regards

                                    Brief Answer:
PEP not indicated
Detailed Answer:
Hi
Dear XXXXXXX
The external meatus or opening of urethra is the smallest part of male Urethra. So it prevents any fluids from directly entering inside the Urethra. Also Saliva is not known to transmit HIV( The amount of viral load is too low to transmit it).
PEP is usually more effective if given within 72 hours of exposure. In your case PEP is not recommended.
I do understand difficult situation you are faced with and with all the fear and anxiety. Let me reassure you there is no cause for concern. Use all the requisite precautions next time instead of being worried and stressed.
Put back the episode and move on.
God bless you

Deepak

Thank you very much!
