Question: Hello Doctor ,

Im XXXXXX . I have a habit of having protected sex with sex workers but one time my condom broke while having sex . This incident took place 8 months ago . Im married and i also have sex with my wife . Everymonth i had test for stds . I have negative for all the stds . I stop testing for stds on january which five month after the incident . Still i have the same habit . Now about 2 weeks ago my wife had pain in the hip and abdomen . So went to the doctor and they did pap smear test and also blood test . In that test got a result that show my wife have numerous coccobaccili . Is this a sexually transmitted disease ? Or is this cause of hiv ? And i have a unusual spot at my penis back from 8 months ago . So my question is that do i got infected with hiv or stds else ? Or it is normal ?

I understand your concern and situation.

As you have mentioned that you practice protection during intercourse. This lowers down your risk of contracting HIV as well as other STD related infections. You reported only one incident when your barrier method was broken. The risk of HIV from that event is very very low. As you have been tested for STD is fine but I suggest you to also take a test for HIV by 4 TH GENERATION ELISA. This will clear all the queries. As for your lesion on penis kindly get evaluation done by dermatologist. It could be STD related.
As for your wife you mentioned her pap smear showed coccobacilli that usually is seen in bacterial vaginosis BV infection. BV is usually caused by a mixed flora, both aerobic and anaerobic. Prevalence is increased in sexually active women affected by other STDs. It is quite contagious, as more than 80% of men are infected when the partner carries an infection. She must take proper antibiotics like metronidazole under the guidance of her gynecologist. She should maintain genital hygiene. It is very much likely that she has contracted the infection from you. Kindly also get her tested for HIV.
I advise you to practice safe sex in monogamy with your wife. Avoid unsafe sex outside marriage.

Hello Doctor ,

I just saw your message . After the incident of broken condom i keep testing for hiv and stds for 5 month it were negative and only hsv1 were positive . So regarding the lesion i taught it were because of hsv1 . So during the 5 month i din involve in any sex with sex workers . From the six month i started again to have the habit again for about 10times or lesser than that . So now i stoped again and i have no intention to repeat again . I dont have any symptoms or unusual myself but ill do the test . Im not worried of hiv .But im worried about my wife . Is this coccobaccilli related to hiv ? This coccobaccilli is curable Or its a life long disease with only treatments ? So what im suppose to do now . She have been adviced to do the urine culture . And probably we shall have the results by friday .

Do we have hiv ?
Do we have been infected by stds ?

Is this curable ?

Please advice me regarding this .

There is no need for you to get panicky or anxious. The disease your wife is having is sexually transmitted disease but it is not serious and 100% curative and not related to HIV. It is better that you have got her urine culture test done. If it has bacterial infection then she will be advised treatment. Nothing to worry. As you told you have got tested for HIV and it was negative is plus point in your case. I advised you both to get tested to clear any doubts of transmission. It is 99.99% that you have no HIV but test can be repeated just for better interpretation.

Thank You doctor . As far now as we dont have any kind of symptoms related to hiv i feel free about it . But ill make sure it by doing a test . Regarding this coccobaccilli as you said its highly contagious , what all should be avoid with us both and also we have a kid ?Does this is transmitted by any means.What you advice about our sexual life, apart from that what are the precautions to be taken . Should i have to do the test something like pap smear , what i have to do now. Is there any natural diet we should maintain while we take the antibiotics .

As I said it is contagious but only sexually to partners. Your kids are not under any threat of disease. There is no need to worry for them. The bacterial vaginosis can be easily treated by antibiotics course which must be prescribed by a local doctor for 10-14 days which will cure the disease. Till that time practice barrier methods for intercourse. Relax and no need of any specific diets. As for as you are concerned you must visit a dermatologist for evaluation if you have any discharge or lesion.

Ok now i feel a bit relief . Regarding the lesion or unusual spots at the penis , i have only have to consult dermatologist or infectious disease doctor ? Since you said coccobaccilli is may be transmitted from me. I also might be infect her with the same again if i dint treat myself . So we need to cure her infection and also mine . But what test im suppose to do . Is it stds is enough or any specific test to be done for me ? So when does we know we free from the infection . Does we need to be tested regularly for this coccobaccilli , hiv , or stds ?

There is no need to go for tests every time. Just once you both need to get tested for STD like HSV 1 &2, Syphilis, Gonorrhea, Chlamydia and HIV tests in a laboratory by rapid tests, VDRL, ELISA OR PCR. After getting results and practising safe sex you do not need to get tested. Yes if any infection is found it must be treated properly under doctor's guidance. You also need to counsel yourself for monogamy. I cannot comment upon your lesion without any examination. So better visit a dermatologist or infectious diseases Specialist.

Hello Doctor ,

As i informed you earlier . My wife been having some abdominal pain and foul smell during urine so that the reason we went to see doctor about 2 weeks ago . They take the pap smear test and blood test . But four days back she started having some pain in left side of lower hip . So yesterday we went to clinic for consulting about this pain but it is not the same doctor who do the test for her about 2weeks ago. So the doctor which we saw yesterday did a scan , she said the urine infection or something is reach the kidney so if you leave it like this then this will lead to stone formation . After that incident only we got the results from our main doctor . The result shows the coccobaccilli .

Now doctor as far your explaination i understood that coccobaccilli is contagious but 100% curable with antibiotics. But as far looking to the scan it shows the infection reaches the kidney and they said white patches on it.Is this because of coccobaccilli Will this get worst situation ? We did the urine culture yesterday , it takes 5 days to get results . I just feel that by day passes if it reaches all parts of the body then it get worst, im i right doctor ?


Is the infection in kidney also be cured with antibiotics ?

I understand your concern. There is no need to worry so much. It seems your wife has developed urinary tract infection and due to reflux or stones there is infection in kidney or it could be infection of kidney such as pyelonephritis. Bacteria from the large intestine, such as E. coli, klebsiella, etc are in the perfect position to escape the anus and invade the urethra. From there, they can travel up to the bladder, and if the infection isn't treated, continue on to infect the kidneys. Treatment is antibiotics for prescribed duration will cure the disease. Drink plenty of fluids. Ask her to empty bladder frequently as soon as she feel the need to go; don't rush, and be sure empty bladder completely, Wipe from front to back, Drink lots of water, Stay away from feminine hygiene sprays, Cleanse genital area before sex, Urinate after sex to flush away any bacteria that may have entered urethra. She will be fine after treatment. It is a common problem mostly unrelated to sexual exposure.

As i understand from you last reply that my wife have a infection in the kidney due to bacteria , How about the white patches showed in scan at the kidney . Is it also because of kidney infection ?.Is this bacteria which cause kidney infection is due to the coccobaccilli which i described you earlier. Is there the kidney infection is because of the coccobaccilli which transmited sexually . As you said the coccobaccilli is contagious will transmit sexually . But this contagious means that coccobaccilli will spread internally and affect the organs ? Because my wife have discharge from 3 days ago and have slight irritation over the private parts . Yes sometimes it is may be due to infection to sexually transmitted diseases as the genital and urinary system are in close proximity. Treatment still rests on antibiotics and fluids. On CT Scan white spots could be due to stones. Kindly consult a urologist for treatment of stones. Stones can cause infection. It is difficult to say if it is same as coccobacilli without culture and molecular tests.

I understand from last reply that you mean that the infection on kidney may also due to the coccobaccilli. So now i have to get treatment after the results of urine culture . Is im right doctor ?

So this means that we have an sexually transmitted disease from coccobaccilli and because of this bacteria my wife have infection in the urethra which spreads to kidney . So will this disease curable at 2 weeks time as you said to take antibiotics ? So it takes long time . If these condition dint treated what hapens please let me know if any ?

Hello Doctor ,

As i informed you that my wife condition with coccobaccilli and infection in kidney . From you replies i understand that it might be due to infection and the cause of infection is due to stds or coccobaccilli . But these condition will get cure with antibiotics . I dint have the urine culture yet probably ill have her urine culture by wednesday or tuesday but today she had a seen a drop of blood in the urine . What im suppose to do doctor ? Now im scared that because of my behavior i have turn her into a uncurable disease which might be due to hiv , syphilis , hpv , rather than curable stds . Your wife had blood in her urine this is called haematuria. This is often associated with burning micturition, frequency, pain in abdomen. It is commonly due to kidney stones or infection. Nothing to panic. Just focus on treatment than thinking about disease. It is a common problem and usually not due to STD. A mid stream urine sample test for microscopy, and culturebfor bacteria (infection). The bacteria is then tested for its susceptibility to different antibiotics. Get a full blood count, kidney function, calcium levels, blood electrolyte levels and blood clotting tests.Urinary tract infection will require a course of oral antibiotics is given. In severe cases, antibiotics through a vein may be needed.Urinary tract stone depending on the size of the stones surgery may be needed to remove the stones.Stay well hydrated. Drink about 3 liters If fluid daily
