Question: Hi i had unprotected oral sex last weekend and was wondering what the chances of HIV transmission were? Also although i had protected sex, she finished with a hand-job, without a condom, and i was wondering if vaginal fluid got on her hand which then touched my penis could this transmit HIV? Does this situation warrant asking for PEP treatment?

1. Oral sex has an extremely low HIV transmission of 1 in 10000 or 0.01 percent. You do not require PEP and it is also late to start PEP of it has happened more than more than 3 days back.

2. For peace of mind you can get the HIV fourth generation rapid test which tests for HIV 1 / 2 antibodies and p24 antigen. This test should be done at 30 days for accurate result.

3. I am sure your result will be negative when you get tested for HIV as oral sex does not effectively transmit HIV and even if she had vaginal fluids (which you are not sure about) then also the risk is extremely low for HIV transmission.

Thank you for your prompt reply.
It is less than 3 days ago. I do not think that I would be allowed use PEP though, as the transmission risk is quite low. I am probably just very anxious and worrying too much about it. In your opinion anyways you do not think PEP is required?

Just to confirm, you think the risk of a handjob with no condom is minimal even if she had vaginal fluid on her hand?

1. Thanks for your clarification on the time duration since the incident.

2. PEP is never indicated for oral sex and you will most likely be refused PEP. Please note that PEP medicines also have their own side effects and they should be taken only when there is unprotected vaginal sex with a partner who is suspected of having HIV infection.

3. Getting anxious is common and I have come across many patients who get anxious even when risk is extremely low. Please relax and tell yourself that there is extremely low risk from the incident and you will get the test done only for peace of mind. The only thing is you have to wait for a month to get the test done for accurate result. However please do not think too much about the test as things will be normal for you

4. Please keep in mind that during the hand job, the semen is forced out of the penis during ejaculation. In this situation it is impossible for any vaginal fluid to go inside the penis opening and cause infection.

5. I confirm that the risk of HIV during a hand job is extremely less unless you had an actively bleeding wound on your penis shaft, which I think is not present.

Thanks again for your reply. I was wondering with what I've told you already and the fact that french kissing was also involved do you think a transmission of Hepatitis B was possible?
I've been experiencing a sore throat, abdominal pain on the upper and middle right side of my stomach (particularly when I breathe in), and a slight pain at the back of my jaw on the right side as well. What could these symptoms indicate? Should I go to a doctor?
