What Are The Chances Of STD Transmission Through Unprotected Sex?

Question: I am worried sick about std test. I've always had generalized anxiety. Recently, out of nowhere i was very scared that i could have an STD, i havent had any symptoms at all. I have had unprotected sex 3 times, one with my girlfriend who i am sure she is clean, and the other 2 with some girls where the condom broke and didnt notice it until we were done. Its been almost a year since my last exposure and still havent has any kind of symptoms of anything. I have decided to take a STD test (full panel) just to be sure.
I need a professional opinion on my situation, since i havent had any symtpoms is it unlikely that i have an STD? I am very afraid mainly on HIV and herpes, i do get cold sores rarely so i know i might have HSV1, is it possible i have genital herpes eventhough i havent had any symtpoms down there?

I am mainly afraid that i could come up with some STD that would end my sex life and my dreams to have a family. I am taking the test today so i am hoping everything goes well.

Would you suggest that this is more of an anxiety case ?

Ok so my first unprotected episode was with my girlfriend about 4 years ago and she was on the pill so we proceeded to have unprotected sex. Second it was a girl i knew and i was going out wirh her for a few days. We only had unprotected sex once and this was about 3 years ago, no symptoms or anything and did not eyaculated inside her (if that matters), i am still in contact with her and if she had something I am sure she would have told me. Third girl (about 3 years ago too) we had protected sex and the condom broke and we stopped, after that I did check myself constantly and never noticed anything. The most recent one was with a girl about my age (25) and we only had an attempt of unprotected sex once since the condom broke, this was about 8 months ago and I am still in contact with her (almost everyday) and she has not said anything. The last girl does not really seem to be the type of girl that would sleep with every guy to be honest, we were dating for a while and when having sex I never noticed anything unsual in her genital area. None of these girls are drug users nor they would fit in the "very sexually active" category. Do you think if I had something i would've presented symptoms by now? To be honest I feel very healthy and I rarely get sick, i am usually more sick from the anxiety itself than from an actual illness from what my family have told me. Still the only way I will get rid of this anxiety is by simply taking the test, should I expect a negative result based on your opinion?

First of all thanks a lot for taking pains in sending all the important details so that I can deduce my interpretation on the basis of the same.

So young man, the good news for you is that based on your type of exposure and your present status, I can not make out any possibility of any type of sexually transmitted infection in yourself.

My firm declaration about your negative status is based on the simple medical and scientific facts that most of the sexually transmitted infections manifests itself within 2-4 weeks and rest if at all within 3 months.

There is always a history of redness, discharge or ulceration and related things which never go unnoticed if have had any STD....which are actually not!!!

In your case interpreting each and every episode with type of partner I can make out that your test result would be completely alright.

So just relax about any possibility of sexually transmitted infections and do give me a follow up of your negative results.

Follow safer sex practice and always use condoms for the best protection.

Yes i will post my result, i already did my test and it should be sent to me tomorrow. What really seems confusing or what really triggered my anxiety was the fact a lot of info out in the internet says that you can have genital herpes and know it, and it seems that it happens a lot, i just want to make sure i am not one of those persons. How true is that?

After reading your complete query, interpreting the facts with diligence I reassure you fully that you do not have genital herpes.

Let me tell you certain important facts related to herpes:

To tell you medically - Herpes lesions manifest itself as small vesicles (fluid filled blisters) which remains for just a day or two and then forms ulcers which are quite painful on genital area. These ulcers then heals in next 5-7 days.

To begin with also these vesicles are quite painful and itchy with discomfort and once the vesicles burst in a day or two then the ulcers which are formed are quite painful for next 4-5 days. So the symptoms never goes unnoticed.
In yourself I can make out that no such symptom is present matching with the above symptoms.

So you can rest assured regarding the rarest possibility of herpes. You can make out that you do not have any of the symptoms which match with herpes or its symptoms or any other Sexually Transmitted Diseases.

It all turned out good, i did however get positive for HVS-1 which i expected since i've gotten cold sores before. I guess i should not worry about it too much right?

My cold sores always appear on my lip in the same spot. I noticed I mistyped the question I had previously about Genital herpes, the thing is that I was reading on the internet (cdc.gov) that almost half people are diagnosed with genital herpes and are asymptomatic how true is this? (at the time I was affraid I was one fo those people... you know anxiety)

I also got another question, I got infected from a cold sore about 4 years ago from a girl friend of mine, the first time was the worse and I even exprienced fever. Since then I keep having them like once a year and sometimes I can go a whole year withouth any outbreaks, I've noticed that overtime they are becoming a lot less violent and last for fewer days. Is it possible that at some point my body will manage the virus and I will stop having outbreaks? I heard that many people get them in their childhood and as they grow up they do not have outbreaks at all I am hoping thats the case for me but I got them on my 20s so I don't know if that makes a difference. I had 2 outbreaks this year since I moved to another city which is extremly cold ( I usually get them when I am exposed to cold weather) I was prescribed Valtrex from a dermatologist.

Would it be a good Idea to take it like twice a year even if I do not have outbreaks just for prevention?

First of all my I am really happy to know that all reports are normal. For herpes 1 it is quite obvious to be positive because your oral sores.

I very well understand your part of anxiety and am with you in all your concerns.

Now regarding your further queries: In herpes it is quite very variable from person to person to get the eruptions and to get cessation of herpes according to the immune status.

The first few episodes are always more aggressive and then it becomes milder in further eruptions.

In some persons I have seen a single episode in multiple years while in others I also see 6-7 or more episodes a year. So it all depends on immunity of a person to whether to get the eruptions or not.

As per your history I can make out good immunity because of less eruptions.... so good news of excellent immunity in you.

For the antiviral medication - valtrex : In general we give valtrex if a person has 3 or more episodes in a year. But in some patients I do use it as a prophylactic dose also even if the episodes are 1 -2 per year. The benefit of this is that the person is at ease all the time form herpes and is non-infective too. But on the other side it must be taken daily to prevent the recurrence. To take a medication daily is quite difficult job for some persons.

So it all depends on your choice whether or not to take a prophylactic dose daily. But once you start then must continue as long as possible.
