Question: My blood pressure is 200/101/84. I took 200 mg of Labetalol 15 minutes ago. I was planning to wait 45 minutes and see if blood pressure has gone down. If not, I have decided not go to the ER unless I have my usual emerging stroke/TIA symptoms (weak, dizzy, numbness or paralysis, severe headache, shaking). I have had several strokes recently and this may or may not be another one starting. Do you have anything else to suggest?

About 10 % of people experiencing TIA, develope stroke in incoming months.

Stroke is a highly potentially life threatening condition, so if suspected, you should be present at your nearest ER until the blood pressure is normalized, especially if you are alone at the moment.

Labetolol should stabilize blood pressure for the moment, but regular blood pressure treatment should be instituted in order to prevent possible strokes/TIA-s.

When this all passes, consult a Cardiologist about modifications on blood pressure treatment.

Thank you for your answer. The Labetelol brought my blood pressure down in an hour to 145/80 where it has remained. I feel shaky, though. I have a dilemma regarding blood pressure medicine. I have orthostatic hypotension and my BP drops 20-50 points when I stand up. Recently it dropped to 80/32 when I stood up as measured by my physical therapost. Most of the time I have normal BP in the 120-130 range. The only time my BP spikes is when I'm about to have a stroke or TIA. So my cardiologist has taken me off all BP medicine on a regular basis and gave me Labetelol only to take in an emergency. I have been to the ER and hospitalized so many times on the last five months that I am reluctant to go unless the Labetelol at home doesn't work. One doctor said maybe my autonomic system is screwed up causing the extreme lows and highs of my BP. What can you suggest to get out of this mess?

In this case, dysautonomia, a condition where autonomic nervous system doesn't work properly, should be evaluated as a possible cause of your symptoms.

There is need to discuss with your Cardiologist about having done 24 hours monitoring of heart rate and blood pressure changes first ( Holter monitoring ).

After this evaluation, the treatment should be adjusted in order to make the changes less dramatic.
