Question : I had protected sex with my girlfriend, I did not ejaculate but when I was pulling out my penis, the condom slipped inside her.

Against all odds, what would be the possibility for HIV to be transmitted if she is HIV+, I am not saying she is, but my mind is deceiving me since this incident

You should know that having protected sexual intercourse, even if the condom slipped inside her vagina when you were pulling out,you shouldn't worry because there is no risk for HIV.
