thanks for your answer. The specifics: Diltiazem HCL 24H 300MG CAP. Now I really am concerned as you say 420mg in a day would be in a normal range and I have now taken 600mg in a 12 hour period. Please advise if I need to go to the ER.

A significant part of it has already been eliminated and therefore you do not have such a very high dose in your system. Maximum doses in clinical trials have been 540 and I believe with the above explanation, you are not above that range.
My advice would be that you monitor closely and report any abnormal symptom of concern. If something new should happen, then I would say seek medical help. But should no symptom be reported, there is no need in going to the ER.
Deep within me, I am convinced you would not need any further medical attention for this. I will advice that you skip the next morning and evening dosages and return to your routine daily dose of 300mg in the next 36hours.
