Mr. XXXX was operated for D12-L1 , L2-L3 Anderson's lesion. D4 to L3 posterior pedicle screw fixation was done in 2015. follow up records are attached for you referrance. once he had mild cocyx paid in 2016 which was treated. recent X-ray shows good fusion & it is assured by doctor that he can perform all activities.
