Posted on Wed, 10 Feb 2016
Question: with I have withdrawals if I change from 10 mg of methodone a day for pain to 30 mg of oxycodone 4 times a day. second question, will I have withdrawals if I change from 60 mg of oxycotin 2 times a day from 10 mg of methadone a day. please answer both questions because I do not know if my insurance will make
an exception to their formulary and pay for oxycotin. Note: I have been taking 30 mg of oxycodone
4 times a day for 7 months prior to the current 18 days on methadone. I want to make sure it was sent to an expert in pain medication in this case
narcotics which were precribed by my physician

Hello, and thanks very much for your premium question.

I am a psychiatrist with expert training in pain medicine.

First, let me point out that oxycodone and OxyContin are the same medication, except one is generic and one is brand name. They both work exactly the same. So, you ask if you will have withdrawal if you convert from:

First: 10mg methadone to 30mg oxycodone 4 times daily

Second: 10mg methadone to 60mg OxyContin twice daily

The equivalent dose of OxyContin converted from 10mg of chronic methadone 10mg use is about 30mg daily. Therefore, if you were already taking 30mg of OxyContin (which you mention you do take) + methadone 10mg daily, you should take a total of OxyContin 30mg twice daily. If I am mistaken and you are only taking methadone 10mg daily, you should take a total of OxyContin 30mg daily. The amounts you mention (30mg 4 times daily and 60mg twice daily) are actually probably too high. In any event, you will not experience withdrawal from either of these doses.
