Brief Answer: Need expanded Information Detailed Answer: Dear XXXX Thanks for contacting Ayurveda with your health concern and following considerations are required from your side or effective Ayurveda Management. 1. any history of chronic constipation? 2. any use of synthetic or fibre based bulk laxatives? 3. any dietary deficiency of sodium [table salt] or under hydration status or excess sweating? 4. Endocrine Disorder like under active thyroid, blood sugar profile or adrenal insufficiency? 5. please specify the type of hemorrhoids whether external or internal? .Because most surgeries leave scars, damage nerves, and affect surrounding tissues, recovery is rarely 100%. No i do not have chronic constipation. I use isabgulla powder. No synthetic fibre. No dietery deficiency. I was diabetic but due to current problem i am eating very less and having lot of fluids and also reduced 15 pounds weight. Currently sugar levels without diabetec medicines are normal. No other disorder. I believe i ate very hot green chilli which created a cut internally and later developed internal hemrrhoids. Actually i had ignored painless bleeding probably that has caused truble. Last four weeks hemrrhoids are gone with hot and cold therapy as well as coconut oil and garlic suppositories. However fissure still not healed. After bowl movement i am in pain burning for minimum 5-6 hours. If i sit on heat paid then i get comfort. I am using currently himalaya pilex tablet and ointment

Brief Answer: Dietary Regime & Lifestyle Management Detailed Answer: Dear XXXX Thanks for reverting back with your clarifications and going by your details following Ayurveda guidelines are recommended -take fresh fruits and vegetables, avoid milk and milk products in diet, excess of tea, coffee, tobacco, alcohol and carbonated beverages. - eat slowly, avoid haste-irregular eating and sweet/heavy food which are difficult to digest and cause indigestion. - take less of strong spices (ginger, garlic), oily-fried-fatty food and flatus producing foods (peas, beans, dals) - take old rice, bottle gourd, moong dal XXXXXXX marich, pomegranate, jaiphal, hot water, adequate rest and sleep. - take fried cumin seeds 500mg, fried asafetida (hing) 125mg, XXXXXXX parpati 500mg, mix them and take with honey three times a day. - 1 glass lukewarm water every morning and night. - to stop bleeding:coconut water from 1 coconut and add 2 table spoon full (tsf) sugar and drink. - drink 1 cup potato juice daily for 1 month. - 4tsf sugar+ 4tsf bitter gourd juice and drink for 1 month - 3 pinches dried ginger+ 2 cup butter milk, drink after lunch and dinner for 1 month - eat 2 tsf plain white butter + 1 tsf sesame powder before lunch and dinner and drink 2 cup butter milk + 3-4 pinches of black salt after lunch and dinner - take: elephant yam, papaya, butter milk, pistachio, almonds, pear, XXXXXXX old rice, milk, crystallized sugar, raw radish and avoid tea, coffee, garlic, onion, fish, hot spices, sitting for long hours. - Trifla because of its high nutritional-detoxification [Shodhan] and cleansing value is very helpful in treating your present complaints. [take 3 gm with XXXXXXX warm water at bed time] PS: . if job demands sitting, make sure you get up every half an hour to relax for 5 minutes and then sit back, make a sponge cushion for your chair. . Again piles, is due to low digestive fire and taking amla & aloe vera juice will help in cleansing the liver to speed up the metabolism. . Do not strain while passing stool, and use warm water or wet toilet paper but without perfume or alcohol(without soap) to clean your anus after each motion . Avoid cold/tap/jet (western seat) water. . .If bleeding is recurrent then take gond katira,(Tragacanth gum) 50gm and make it into fine powder, take this powder everyday, 6 gm in 125 gm curd in morning after breakfast. (you can check Google for better identification of the medicine) PS. since knowing our limitations, Dietary Regime and Management has been provided from my side, but since you were examined by a surgeon [proctologist] who is in better position to explain the prognosis be in his regular guidance. Thanks for advise, however I need a few clarification : 1. You have mentioned in one point that milk and milk product should be avoided and in another point you mentioned that milk is fine. Should I take milk or not? 2. What is XXXXXXX par pati? Will I get in USA? 3. Gond katira. Is it normal Gond which we get at grocery store or different? 4. What I should apply locally to reduce burning and pain? Currently I M applying Himalaya pilex ointment.

Brief Answer: Clarification Detailed Answer: Dear XXXX Thanks for the concerns raised and below are the details A. 1. butter milk [लससी] is advocated since it contains probiotics which is soothing to stomach and skin [improves immunity when taken regularly] PS. 1. One cup of whole milk contains 157 calories and 8.9 grams of fat whereas one cup of buttermilk contains 99 calories and 2.2 grams of fat. [due to churning effect] 2. In Ayurveda we hold Piles/anal fissure to be due to low digestive fire and since butter milk has digestive properties you can take it along with ingredients like asafoetida, coriander leaves, ginger, currey leaves and sea salt to enhance its digestion-aiding properties. 3. Ayurveda medicine considers cow’s milk to be a strengthening, nutritive driver for medicines [Anupana] thus do not take it individually but as a vehicle for administration of drugs, since it causes increase in number of CD4 helper cells [if your Attending Doctor there prescribe you medicine to be taken with milk] B. 1. XXXXXXX parpati is an Ayurveda medicine generally used/given in urinary disorders but since it is very good for Digestion [दीपन-पाचन] thus given in such conditions. 2. It is manufactured by XXXXXXX Baidyanath and hopefully will be available there. C. 1. although this has to be normal Gond, but since country is different, your Grocery shop will be more efficient in handling the same. [make sure they differentiate it from guar gum] D. 1.for itching/pain you can go for "Sitz bath" i.e. soak your anus in bath tub filled with hot water for 10-15 minutes 2 or 3 times a day, if you experience any anal spasm then you can opt for alternate hot and cold bath every 5 minutes for half an hour. [which you have/are already performing] 2. apply Jatyadi tail [shree baidyanath] around the anal region (which will promote healing and also helps in relieving the associated itching) 3. apply lignocaine ointment 5 minutes, and pilex ointment 2 minutes after that [before going to washroom] and repeat the same after your activity is finished. Any suggestions are welcome Dr. Munish Sood Naimittika
