Question : Hi! I just had a massage that included genital stimulation and am a bit worried since the massuesse had a cut on her finger. Could you please let me know if I'm at risk of anything and what I should do? Thanks a lot!

Dear Dr.Chail,
I am not sure if she applied hand sanitiser before the procedure. It is the usual procedure but I didnt see it, so there's no guarantee tthat it happened in this case. Basically, during the massage I noticed that she put on a bandage on the back of her index finger, that covered the entire finger. I'm not sure if the wound was freshly acquired or whether it was there before. There was no blood that I could see, however there may have been some small blood spilling that I couldnt see.
My fear is that if the wound was freshly acquired, the bandage may not have provided sufficient insulation, or that there may have been some blood spilling which I could not see. Essentially there was no visible bleeding, but I couldnt see her hand before she put on the bandage. Sorry for the inadequate information! Please let me know your advice! Thanks once again

I guess all masseurs are instructed to use some form of hand sanitizer before serving a new customer. Anyways, if the bandage was at the back of her index finger then the wound was probably some hours old. Blood clots within a few minutes (less than 3) and most likely she was not bleeding actively. Therefore risk of HIV is almost nil. An important point will be what exactly did she perform on you, I mean your do not have any cuts on your penis right? then there is no risk of any active infection entering your body.

Concerning other diseases, Herpes and HPV are transmitted by close skin contact, especially if one of the people involved has rashes and blisters. I suppose this risk is the same as any person getting a massage and having a cut does not make a difference.

Dear Dr.Chail,

Thanks very much for the reply. I do have a couple of final questions. Since she applied the bandage during the procedure, its possible that the wound was fresh. in this case, is it possible that some blood may have entered the penile urethra? How much blood is required to enter the urethra to pose a risk of infection? And how long does it take for HIV infected blood to die, once exposed to oxygen? I did not have any cuts on my penis as far as I can see or feel. Therefore if there is any risk, it is by the urethra. I read on a similar question that such a situation only poses a risk if both individuals have cuts. But isnt the urethra a mucous membrane capable of transmitting the virus? Could you please explain how the urethra does not pose any risk in this case?

Would you estimate that the chances are so small that it would not require a blood test? Thank you once again.

In my opinion it is unlikely that blood might have entered the penis urethra. In males, the greatest risk of HIV infection is due to vaginal fluids entering the urethra under pressure during sexual intercourse due to the penis coming in contact with force. As described by you, there is hardly any pressure on the urethra and she did not have enough blood or body fluids during stimulating your genitals. It is true that cuts on both individuals carries a greater chance of HIV transmission.

As the urethra is a one way passage for semen and urine, the possibility of the virus entering it against the flow is least likely.

I certainly would like to re assure you that you do not require a blood test but if it is troubling your thoughts then I suggest you get a HIV screening test done at the end of 3 or 6 months for peace of mind.
