Posted on Thu, 9 Oct 2014
Question: Dear Sir/Madam,
I admitted my 3 years (weight 15kg) old son to hospital on 24th Apr'14 due to convulsion(fits) and doctor released him next day after a city scan of brain. Scan report was ok as per doctor. After that local doctor suggested that whenever his fever cross 100f we have to give him parasitamol and 1 and half tablet of Frisium-5 to protect convulsion(fits).
My question is frisium-5 is really require every time when fever cross 100f and is there any side effect of Frisium-5.

Brief Answer:
Intermittent Prophylaxis required

Detailed Answer:
Hi
Welcome to Healthcare-Magic
Greetings of the day

Dear Mr XXXXXXX

Your child is having Febrile Convulsion. That is every time the child has fever especially more then 100f, there is risk of having convulsion.
Normal CT scan is very reassuring.

So objective of treatment is
1. Rapidly control any fever due to any cause by giving antipyretic like Paracetamol: Normalizing the temperature reduces the risk for convulsion.

2. Short term use of anticonvulsants: Clobazem is preferred and is usually given for 3 days. It reduces the risk of convulsion

Clobazem is a Benzodiazepine group of anti-convulsant(drug used to prevent or treat fits). It is on elf the safest anti-convulsant and short term use is free from any major side effects.

Every time your child has fever , i. would suggest you the following

1. Immediately give antipyretic like Paracetamol.
2.Fresium tablet to be given for 3 days during the episode
3. Tepid sponging: 15 minutes after giving Paracetamol wipe the whole body with normal tap water at room temperature. It will help in reducing the temperature rapidly

majority of children (97%) improve and do not have convulsion after age of 5 years. So it is important to follow this Intermittent prophylaxis at least till age of 5 years, after which risk of convulsion is drastically less.

Do get back to me anytime for any further assistance.

Dear Sir,

Local doctor suggested for Paracetamol when fever touch 100f and for tepid sponging when fever cross 102f.
Please suggest if it is ok or not.

Also suggest what should be the preferred dose of frisium-5, one and half tablet twice in a day is require or one tablet twice in a day is ok for 3 days.

Lastly what is the best paracetamol for him presently we are giving him Calpol 250mg (5 ml each dose), and what should be the minimum gap between paracetamol dose, because whenever fever comes its touch 103-104f even few times it cross 104 also.

I am really worried about him, kindly help.

Brief Answer:
Do not worry

Detailed Answer:
Dear Mr XXXXXXX

Being a parent myself , I understand your concern and apprehension.
Tepid sponging helps in bringing down the temperature back to normal. So irrepestive of degree of fever it still helps.

Calpol is a good brand of Paracetamol. You can give Paracetamol in dosage of 15mg/kg per dose every 6th hourly. % ml can be given 4 times a day).

The dosage of Frisium is 1mg/kg/day. So you can give 1 1/2 tablets twice a day for 3 days during episode of fever.

Do not worry.
Keep me updated.
I pray he does not have any future episode of convulsion.

Regards
Deepak

Brief Answer:
Nothing to worry

Detailed Answer:
Hi
Dear Mr XXXXXXX

As it was a clearly documented case of 1st time Febrile convulsion and there has been no further episodes : It is very reassuring.
EEG may show some changes especially during sleep. It does not need any long term anticonvulsant prophylaxis.

The aim is to rapidly decrease the fever every time it comes and to give anticonvulsant (Frisium) for short duration (3 days). If the child still has convulsion in spite of taking precautions then long term anticonvulsant therapy is required.

Rest be assured presently the child does not need any further intervention.
