Question: I experienced an allergic reaction to Omnicef last Tuesday. As the rash was leaving, my hands became quite painful. Do you know of this phenomenon? They're both very stiff and painful.

Brief Answer:
SEems neurotoxic effect of the antibiotic

Detailed Answer:
Hi
Thanks for posting your query on HealthcareMagic.

I am MD pharmacology and I will try to help you as best as I can.

Antibiotics in general can cause neurotoxicity. Omnicef belongs to a group of mediicnes called cephalosporins. Toxins released during their use can penetrate nerves and blood brain barrier and cause neurotoxicity. It is comparatively an uncommon side effect.

In your case it seems to be peripheral neuropathy and your brain is not affected. I believe that stronger painkillers would be effective. Painkillers used in other neuropathies should be tried like low dose gabapentin, low dose amitryptiline, opiods, etc.

Like other side effects of medicines, this neuropathy is likely to disappear spontaneously in some time. The duration may range from few days to few months.

Hope it helps. Let me know if I can assist you further.

Brief Answer:
good health

Detailed Answer:
Hi,
Thank you for query.

Reviewed your reports. Your sugar levels are high and your creatinine level is slightly high.

Are you diabetic?
You need to check your fasting post prandial sugar levels and HBA1C to rule out diabetes.
Repeat your creatinine once again as said by your doctor,Because even dehydration can increase creatinine level.

Meanwhile drink plenty of fluids.
Go for regular walking at least 30mins.

Regards,

Yesterday was 4 weeks dealing with this. I saw an allergist who believes I've developed Serum Sickness from the allergy to Cefdinir. Today we're doing scratch tests to see what antibiotic I can take in order to get my dental work completed. My hands and wrists are still involved, but are slowly improving. The Dr may order 1 more round of steroids (#5), but high dose this time.

Thanks for your continued concern.

Brief Answer:
Serum sickness presents with rash and arthralgias

Detailed Answer:
Hi again

Serum sickness is an immune complex disease that presents with rash, fever, arthralgias, lymphadenopathy, glomerulonephritis.

Need to know the exact location of pain in hands. If your joints are painful than it is called arthralgia and is a symptom of serum sickness. If your hand is painful on touching the skin, it is more probably a neuropathy. Their management is different and treatment must be taken according to diagnosis.

Diagnosis of both serum sickness and neuropathy is mostly based on clinical examination.
In serum sickness, the following laboratory findings maybe there like low platelet count, raised erythrocyte sedimentation rate, raised C-reactive protein, sometimes protein and haem may be seen in urine. Kidney function tests may be abnormal. Serum sickness usually resolves in 4 weeks. If its lasting longer, other diagnosis should be sought.

Since steroids didnt help much, I doubt if they would be helpful in 5th round.

Many dental treatments dont require antibiotics at all.
