Question: my doctor placed me onSovaldi and Ribivirin i was hgaving chest pain s myt arms were going numb He did not mention any of these thing in my medical reports he just said hang in there concentrate on the cure not the side efffects i was to take them for six months He retired four months in to the regimen leaving me in no care with any doctor I threatened to XXXXXXX his colleague said he would not take me as apatient reluctantly he agreed he looked over my charts said I was cured and said you dont need to take the medication anymore your cured i still had two momnths of meds left He was the doctor i quit taking them the virus came back three months after he told me to stop

You are having chronic Hepatitis C virus infection and have been treated with IFN-α and a combination of Sofosbuvir ( Sovaldi) and Ribavirin for a period of 4 months but you failed to have sustained virologic response (SVR).

Please let me know the genotype of HCV virus (genotypes 2 and 3 favorable as opposed to genotypes 1 and 4) and baseline HCV RNA level.

Therapy for those with genotype 1 should last a full 48 weeks, whereas in those with genotypes 2 and 3, a 24-week course of therapy suffices(although refined tailoring of treatment duration may be indicated based on rapidity of response or associated co-factors).

Features Associated with Reduced Responsiveness:
Genotype 1a (compared to 1b)
High-level HCV RNA (>800,000 IU/mL)
Advanced fibrosis (bridging fibrosis, cirrhosis)
Long-duration disease
Age >40 years.

Usual recommendation for Nonresponders to a previous course of standard IFN monotherapy or combination standard IFN/ribavirin therapy: A course of PEG IFN plus ribavirin—more likely to achieve a sustained virologic response.
