Side effects occur due to high dose of Methotrexate. The following ones are related to bladder and vagina:

1) Bladder inflammation and cancer (in long term use).
2) Renal impairment and hematuria (bloody urine).
3) Vaginal pruritus (itching) and infection.
4) Vaginal bleeding or spotting.

Folinic acid supplement is given as antidote of Methotrexate toxicity. If the patient suffer from any of these symptoms, it is worthwhile to consult the treating doctor and ask for folinic acid supplement.
