Your BMI is alright as per your age group. However, the cholesterol and LDL (the bad cholesterol) levels are just borderline elevated, along with some secondary changes in the hematological profiles and insufficient Vitamin D3 levels.
Stop smoking/using any form of tobacco completely (it seems like you do), better to avoid alcohol also (if at all consume)...drink plenty of water throughout the day(at least 2.5ltr per day)...
Apart from that, you should also initiate some dietary modification on daily basis. Try to avoid junk/fast foods completely, no excessive deep-fried foods strictly...also make sure to drink enough water throughout the day (at least 2.5ltr per day)...

Make sure to get daily sun-exposure on bare body parts for around 15-20min in the morning, preferably in between 8-10am (it'll help in natural synthesis of Vit-D levels)...include foods like fortified milk, almonds, pistachios, hard-boiled eggs, spinach, orange juice, plenty of legumes to maintain proper Vit-D3 levels in the body...
Follow all these strictly and recheck the CBC and Lipid profile, 12 lead ECG, serum calcium, Uric acid, Iron profile after 3months and also get a clinical check-up done by any Internal medicine specialist...This should be enough for the time being for the present as well as future well-being (along with once yearly comprehensive health check-up plans)...

That's mostly secondary changes due to year old bad habits...Just follow all the advice given earlier, stop smoking/using any form of tobacco completely, avoid junk/fast/processed foods...eat healthy diets, drink plenty of proper mineral/R.O purified water only...things should reverse in a few months...
