Question: HI. I was sleeping the past couple nights and Ive been having a large amount of clear fluid coming out of my nose. Tonight, it also came out of my mouth.
I have been diagnosed with achalasia and GERD.

Hello and thank you for your question.
The clear fluid from your nose could be from allergies. If it is from allergies you probably have sneezing and some clear fluid from the nose in the daytime. It would be helpful to take an antihistamine like benadryl at bedtime if you think you may have allergies.
It could also be from the achalasia and GERD.
GERD which is made worse by the achalasia often causes something called water brash. This happens when acid from your stomach refluxes up into the mouth and causes a spontaneous discharge of saliva from your salivary glands. The best treatment for this is treating the GERD with medications. It may also help to raise the head of your bed about 3 inches. Propping your head up on a pillow does not help as much as raising the head of the bed.

Is it possible that this causes me to choke and cough while sleeping? It feels as if I'm drowning under water

Hi, Yes. GERD is a common cause of cough and with achalasia it can be even worse. Taking a proton pump inhibitor like omeprazole can help but at night it is really critical for you to raise the head of your bed to prevent this fluid from choking you and making you cough.
