Question: Hello Dr, please find the attached report and let me know ur thoughts on how to proceed?
Also, her Creatinine level from last month has increased from 4.39 to 5.71 and there is a slight increase in urea as well. What can be done to bring down the creatinine level and urea? can u suggest any diet plan along with medicines?
Medical History: She is diabetic for last 20 yrs. Last month she was diagnosed with cyst in the urinary bladder. So, we have done Cystoscopy TURBT and sent the tumor for investigation. It turned out to be a high grade cancer. I have attached the biopsy report as well.

Hello,

Your mother in law has been diagnosed to have Chronic Parenchymal Kidney disease secondary to Diabetic nephropathy and high-grade Cancer of Bladder

As regards her recent raised Creatinine levels of 5.7, I would suggest you be under constant supervision of qualified Nephrologist at regular intervals.

Reducing protein and salt intake to optimum levels and control of her diabetes are the main key factors in the management of Diabetic Nephropathy.

Considering the high-grade cancer of bladder which has a tendency to spread faster there is nothing much that can be done but to offer her a major surgical procedure like Radical Cystectomy at this age.

I would suggest you consult a qualified dietitian to chart out her food intake planning that will suit her health. Dietitian would be the best person to chart out her diet, vegetables, type of fruits, protein intake as per her daily caloric needs and to keep her creatinine and urea level at reasonably accepted levels.
