What Can Be Proteinuria?

A.     Brief history about pateint
a.     31 year male , 108 kg , bp slightly elevated as 130-140/85-90
b.     Some health conditions –
have localised scalp psoriasis since last 8 years , have obesity since last 5 years , heel pain since last 6 months (doctor said it is planter facilities no swelling) , proteinuria since 1 year

I had chest pain 6 months ago then I got tested, heart doctor said your heart health is ok but you have proteinuria. Then I met nephrologist. He suggested my KFT and electrolytes, egfr are good but have proteinuria. He suggested to lose weight.
I lost 8 kg in 1.5 months with extreme dieting.

c.     Current problem –
I am attaching my health reports .
Now I feel joint pains , muscles soreness, muscles pain , after losing weight I was not able to sit too long as my bach aches , then I again increased my 8 kg weight ,

after sex I feel too much muscles soreness all over body , joint pain , joint sounds when folding unfolding and all over body pain .


B.     My query related my health
1.     Is it normal to have proteinuria if KFT is normal?
2. Pain in body after sex is side effect of dieting and nutrition deficiency or other issues like kidney damage ?
3. Medications if recommended

Thanks for your query,

Though, the results of KFT presented by you are in almost normal range. Still, there are some concerns, which are to be taken care of.
-Though mild proteinuria may be very much normal, but if proteinuria is persistant more than 3 months, it should be further investigated. Protein more than 300 mg/gm of creatinine are abnormal values, need to be evaluated further. Hence, you need these values for proceeding further. Value of eGFR is also needed, to asess the kidney functions.
- Both, your calcium & vitamin D levels are low, which need to be investigated further for the reason & treatment accordingly.
- Loss of protein in urine as well as decreased levels of calcium & vitamin D may be cause of your weakness & fatiguability.
Here, I advise you, to get yourself properly examined, investigated under guidance of a nephrologist & treatment done accordingly.
Also, a word of caution, never do a crash dieting. The weight loss should be gradual & not more than 2-3 kg in a month.
For the time being, I will advise you not to take any additional medicine before getting yourself properly evaluated. Help of a qualified dietician may be taken for maintaining your weight.
