Question : What can cause blood to be in one's semen?

Most common cause of blood in semen are injuries or structure of urethra, infection of lower urinary tract.
Other causes are bladder stones, prostatitis, malignancy.
I would like to know following:
1. When did you notice it first?
2. How many times have you noticed this?
3. Any burning in urine?
4. Any history of unprotected sexual intercourse?
