Question: I am writing for a male 33 years old, He has been dealing with an eye that swells shut randomly, just the one eye, for almost a year. it will completely swell shut, he has had allergy tests , blood tests, eye MRI, no one can find anything. The eye swelling does not seem to be related to to any particular activity, however sometimes seems worse after excessively long day... just wanting to know where to look next??

Brief Answer:
Instant swelling of the eye goes well with allergic reaction- further explanations below

Detailed Answer:

Hello,

By what you explain it seems that the most likely cause of the instant eye swelling is due to allergy. It causes quick swelling but no eye pain and no vision changes. Blood tests can come out normal, even allergy tests might come out normal and he still might have allergic conjunctivitis.

I would recommend him to:

- use oral Loratadine followed by,
- eye drops with Ketotifen to reduce the allergic reaction. They are well tolerated but the patients with minimal risks for side effects.
- advise him not to rub the eyes as this makes swelling get much worse
- use or local cold compresses can help reduce swelling and quiet local symptoms in the eye
- he should notice an improvement in a day or so and if no improvement with these measures or any worsening he needs to be seen by an ophthalmologist
- if he gets swelling of lips when he gets swelling of the eyes than more serious allergic reaction should be suspected and he should consult his local doctor for further care.

Hope I have answered your query.
