Hello,

Welcome and thanks for posting your query to the forum.

I can understand your concern for the dry flakes. It appears from your query that you have the condition of fungal infection and after application of the cream the area got dried up to produce the flakes. It is nothing serious and is quite usual after application of steroid cream XXXXXXX to rebound infection.

Even then in view to help you best I humbly request you to kindly send me few good resolution pictures of the affected area especially focusing the patch.

Hello Doctor,

Thank you for the timely response.

let me give you a comprehensive explanation about flow of events.

1. The itching was on left groin area ( the are adjoining thigh and pelvic region). combined with dryness on shaft with flakes

2. if you observe the pics there is a dry pimple like patch on shaft. even on testicles yesterday I had similar dry patches with flakes falling when rubbing. something like when we have a injury and it dries and dead skin falls off.

3. these is really making me nervous Std is back in my mind though the last time I had proper sex in 2012.

Please help me with next steps.

Thank you.

Brief Answer:
Do not worry it is simple fungal infection. No STD

Detailed Answer:
Hello,

Welcome again and thanks for posting your details and pictures.

After reviewing the details and pictures let me reassure you that you are suffering from fungal infection at scrotum region. It is a recurrent type of fungal infection which is occurring again after the use of steroid cream (clobetasol).

A steroid cream should never be used as it causes recurrence and dryness of skin after recurrence.

To tell you about the Fungal infections: These are common type of infections which can affect anybody at any site. But more commonly it affects the areas which are more prone for sweating or moist areas (as in your case). More specifically these are related to the hygiene and environmental factors.

Due to hot and humid climate or due to tight and synthetic clothing,it leads to growth of fungus at any part of the body.

It has no relationship with any sexually transmitted infection so do not worry aobtu any STD.

For the betterment kindly follow the suggestion below:

- Keep your self away from hot and humid climate.

- Take two times bath.

- Wear cotton clothes and cotton undergarments to be changed two times a day. Do not wear any synthetic or tight garments.

- Do cold compression or warm water compresses according to the weather condition.

- Do not sit continuously for long hours.

-I suggest my patients to apply anti-fungal creams such as "Clotrimazole" or "Sertaconazole" or "Amorolfine " and take anti-fungal tablets according to medical advise.

-For itching I suggest oral anti histamine like loratadine or levocetrizine two times a day under medical advise if there is moderate to severe itching is present.

-Don't try to scratch or rub the area as can lead to further aggravation of the problem.

- You can use any good antiperspirant powders daily to avoid future infection.

- Reduce your weight if it is on higher side.

You can follow the above regimen for 3-4 days and if benefit occurs can follow the same for next 10-14 days.

I hope these information's will help you. If you have further queries I will be happy to help.

"Wish you good health and early recovery from the problem"

Brief Answer:
Fungal infection: antifungal & oral anti histamine

Detailed Answer:
Hello young fellow,

Welcome again to the forum and thanks a lot for your swift follow up.

IN my patients I prefer to give anti-fungal tablet such as Terbinafine 250 mg (Terbicip) for once daily for 3 weeks of period.

For itching I suggest oral anti histamine like Des-loratadine 5 mg (Dazit) two times a day as long as the itching or irritation is present.

Rest of the suggestions as are as before. I reassure you again that your condition is simple infection and can be managed by better hygiene measures.

I hope these information's will help you. If you have further queries I will be happy to help.

"Wish you good health and early recovery from the problem"
