Question : Oh, yes! My husband is 70 and has a painless swelling in one testicle. The testicle is much larger than normal. It has been swelling slowly for 6 months and he never said anything. I have to say I am quite upset, and he will finally see his doctor on the 30th of this month. XXXXXXX

Brief Answer:
Exclude testicular tumor, urologist s evaluation

Detailed Answer:
Hi and thanks for the query,

It is imperative for him to get a proper medical consultation. This could be benign, as well as serious sometimes. Most testicular tumors are generally painless and do present as such. Other signs and symptoms however should be watched out for like enlarged lymph nodes around, fever, weight loss and an altered general out look. In some persons however, even with testicular tumors, they might remain without symptoms, though the tumor might already be spreading.

Blood tests like Human Chorionic Gonadoptrophin, ACE levels, Bone blood Alkaline phosphatase levels, a testicular ultrasound could be very important. A chest X ray is of utmost importance since this is a main predilected zone of spread of these tumors.

Other causes of welling like cysts, inguinoscrotal hernias should be considered.

I suggest a urologist opinion a keen evaluation immediately your family physician confirms the mass is mainly testicular and it is not a cyst. Early diagnosis and proper management and key factors affecting. Other causes could lead to painless swellings, but excluding that it is not a testicular tumor remains a priority.

Kind regards,

DR Bain
