What Causes A False Negative For Butrans Transdermal Patch In A Urine Drug Test?

default
Posted on Thu, 23 Jul 2015
Question: I just started the butrans 5mcg/1hr about 3 weeks ago, just started my 4th patch. Today my urine drug screen came out negative. I have not missed one patch, some sunless spray XXXXXXX was sprayed over it early this morning before Drug Screen at pain management . Could that effect my patch? I have not missed a patch !
What else could cause it to be negative?

Brief Answer:
many factors can cause false negative resutls, upload test reports

Detailed Answer:
Hello,

Thanks for writing to us.

Spray mostly does not affect patch but still I want to know what are the contents of the spray how much quantity did you spray directly over patch.?

Buprenoprhine(opiate) in general detected up to 72 hours in urine test from last dose.
One possibility is that you drug concentration was low which can not be detected in urine.
Other possible reasons are :

- Dilute urine (excess fluid intake, diuretic use)
- Metabolic factors
- Inappropriate test used
- Elevated urine lactate

Buprenorphine requires an expanded immunoassay panel for detection.

-Let me know any of the possibilities from above reasons.

- Upload test reports.

- When did you apply last 4 patches ? Describe me exact dates.

- Are you taking any other medicines?

I will be happy to help you after your response.

I take 1 mirapex 25mg at 9pm daily
Also 2 flexeril 10 mg daily, one at 6 am one at 6pm
I do not have test report, the NP showed me where it show 0 mcg,
The patch Butrans 5mcg/ hr.
1st patch 6/9/15
2nd patch 6/16/15
3rd patch 6/23/15
4th patch 6/30/15
The spray XXXXXXX was from XXXXXXX and Go and its their mistic XXXXXXX
It sprayed lightly over my hip twice, nothing heavy then a dryer dries it .
You stand and move as it sprays and dries. Like front is sprayed then dried with A blower then left side,( patch on backside top of hip) sprayed and dried , back , then right side sprayed and dried. Finished, so with the location it was misted with the sunless tanning twice) Im not sure of what it contains.

Brief Answer:
which test is used that matters

Detailed Answer:
Hello XXXXXXX

Thanks for information.

First of all butran has no drug interaction with mirapex of flexeril which can reduce buprenoprhine level.

I checked http://darktansalon.com/mystic-custom-blends/.
You misted the spray twice on around/patch. Contents of the spray are various. Many of them are herbal products.
One possibility is that this spray had done something with patch and reduced/stopeed delivery of buprenoprhine to skin. And that is why your test level is 0.

If you have removed last patch on 6/30 and applied new one on the same day some buprenorphine must be there in your urine for at least 48 hours. In that case possible thing is test method used is not so much sensitive to detect buprenorphine with this much low level.

In this situation the best thing is to go for testing on 4th day of applying the patch as after 72 hours drug attains steady state. You should also confirm with doctor that application of patch is proper and site is proper.

In any case it matters with your pain. Screening done I think to just check that you are not misusing/abusing product and your level must not be higher than prescribed limit. So no need to worry much.

Feel free to discuss further.
