Question : I had a head massage followed by a nose bleed. Should I be concerned?

I have gone through your query and would try to help you in the best possible way. There are tiny extensive blood vessels near tip of the nose on inner aspect) which may bleed if there is any trauma & usually stops itself. You can apply compression to stop it as well. I would suggest you to wait & observe. If bleeding occurs again then you can visit hospital & get an appointment for an ENT specialist for examination and further workup.
