Question : hi, i am obese and have lymhdema in my legs. this is a rash behind my knee where the fat overlaps. its usually just like a yeast infection from sweating but this seems a little different but maybe because its just really bad this time. i didn't know right away. what do you think it is?

It is possible , based on the history given and the images, that you may have a combination of infections along with intertrigo (inflammation of the body folds).

Heat, friction and moisture tends to occur in the body folds- this cause intertrigo which is inflammation of these folds.

Intertrigo can be complicated by fungal/yeast and/or bacterial infection.

There may be intertrigo (inflammation of the fold) along with fungal and bacterial infection. All of these would contribute to the redness and discomfort that you are experiencing.

Treatment: (1) when you are at home, the folds need to be separated by linen or cotton material to reduce friction and as much as possible they need to be exposed to air

(2)Absorbent powders such as zeasorb

(3)creams such as triple paste or XXXXXXX goo can be used as barriers
Triple paste contains zinc oxide, petrolatum and aluminium acetate (this acts as a drying agent). An anti fungal powder or cream and antibiotic cream eg mupirocin would need to be added to this

Antifungal creams include econazole, miconazole, ketconazole
XXXXXXX goo-contains nystatin powder (anti fungal), hydrocortisone powder (the mild steroid reduces the inflammation) and zinc oxide (barrier).
An antibiotic cream would need to be added.
