Brief Answer:
Detailed clinical examination and investigations.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/46 - pain in abdomen and back pain on eating - on a scale of 10/10 - has to go to bathroom immediately - liquid stool - very weak - dizzy sometimes - fever - going on for couple of years - chronic pancreatitis - mesh for hernias - abnormal cervical cells - very distended and very hard off an on upper abdomen...
Since your problem is long standing, I hope you have undergone many investigations; please post all the reports.
The probable causes are:
- The pain in the back on eating indicates two major issues: Gastric ulcer and/or pancreatitis with stones in the pancreatic ducts.
- Immediate rush to the bathroom with liquid stool can be a reflex phenomenon.
or there can be adherent transverse colon on the pancreas due to old pancreatitis and hence the immediate action.
- The resultant effects are dizziness, weakness and fever
- All these things and may be intestinal obstruction causes the hard distention of abdomen in the upper abdomen, can there can be pseudocyst of the pancreas.

I would advise you the following:
- MRCP for the liver, biliary tree and pancreas.
- Upper GI Endoscopy and with Endo US that is endoscopic ultrasound.
- Contrast enhanced CT scan of the abdomen and /or Barium Enterography/ Enteroclysis.
Clinical evaluation by a Surgical Gastroenterologist to assess the clinical picture, correlate with the investigations and take the further action as per the final diagnosis.
Tests of blood: CBC, liver and kidney functions, HbA1c, sugar, vitamin deficiency and correction.

I hope this answer helps you to get an early and proper diagnosis and gives plan for further management to improve quality of life.
