What Causes Abdominal Bloating Along With Severe Heartburn?

Question: Hello.
My stomach bloats after almost any meal, sometimes I have burning feeling in my stomach after I eat and burping (I had that problem for the past two years after certain food but now the bloating is almost all the time) I have burning/sour feeling in my mouth all the time for the past month and recently my eyes sting especially in the morning. Could it be lpr? I don’t have a heartburn. Also the results of ANA and anti dsdna are positive.

Your symptoms are suggestive of Dyspepsia with GERD(LPR).
Basically dyspepsia results when the acid of the stomach comes in contact with the mucosa of the digestive tract. These acids cause a breakdown of the mucosa leading to inflammation and irritation, which leads to indigestion. It may also result from eating disorders or certain medications.




Stomach is lined by a natural layer of mucosa membrane which is resistant to acid however mucosa in esophagus is easily destroyed by acid which regurgitates back from stomach.This cause severe irritation of esophagus leading to inflammation causing severe burning sensation in mouth .Reflux of acid is responsible for sour taste in mouth.Its not necesssary that heartburn is always there in every patient.

The esophagus is a muscular tube that is located in the chest and serves to transfer food from the mouth to the stomach. The lower esophageal sphincter (LES) is a valve that is located at the junction of the stomach with the esophagus. Its function is to prevent acid and other contents of the stomach from coming back into the esophagus. GERD is a condition in which excessive acid-containing fluid refluxes (flows) back into the esophagus, in part because the lower esophageal sphincter is weak.
In some patients if not controlled properly , the esophagus reacts to the repeated injury from the acidic fluid by changing the type of cells lining it from squamous (normal cells) to columnar (intestinal-type cells). This transformation, called metaplasia, which increases the risk of adenocarcinoma by 5%.
Therefore it should be controlled properly.

Bloating and excessive gas -Bloating and acid can be caused by any kind of food. In most of the cases, it's fatty foods which cause these symptoms. Any food whether it's fat or protein or complex carbohydrate when can not be properly digested, releases large amount of gases like hydrogen, methane, carbon dioxide which causes burping.

Now management consists of two step-
1)Control acid production-Proton pump inhibitor like Rabeprazole 20 mg once daily should be started.
2)Prevent reflux of acid formed back to esophagus- Domperidone 30mg or Levosulpiride(both are prokinetic)slow release once daily. This will slow down the reflux of acid back. Prokineic should be added in your regimen.

An antacid containing local anesthetic (Mucaine gel ) should be taken 2tsf thrice daily.

Acotiamide is another wonderful new drug and is very effective in controlling symptoms of GERD and esophagitis. In trials- Acotiamide, a gastrointestinal motility modulator, at a standard dose of 100mg thrice daily has significantly affected esophageal motor functions or gastroesophageal reflux in healthy adults.

All these drugs should be given by your gastroenterologist to provide you with relief in your problem.

Avoid-
Dairy products, which contain sugar lactose that causes gas.
Vegetables, including onions, radishes, cabbage, celery, carrots, brussel sprouts, broccoli, cauliflower and legumes.
Fruit sugar, which is especially high in prunes, raisins, bananas, apples, apricots and fruit juices from prunes, grapes and apples
Fiber.
Fatty foods and carbonated drinks.

Try Eating more fermented foods. These are rich in both good bacteria and enzymes you can try raw natto kefir or cultured veggies. This is probably one of the most important first steps.
Take a high-quality probiotic.
Take external enzyme supplements.
Exercising, to help keep food moving through your system.
Using spices that may help to prevent flatulence. These include turmeric, coriander, peppermint, fennel, sage, chamomile and ginger.

I understand there is a difference between GERD and LPR. Which may I have according to my symptoms? Which doctor should I go to to comfirm? Is eating honey ok?

Your symptoms are suggestive of GERD.
LPR is usually involved with vocal cords inflammation hence a change in voice is there.The term LPR is used to describe the acid in the stomach that comes up into the throat at the level of the laryngopharnyx. LPR causes irritation and changes in the larynx. GERD is cause by the backflow of gastric contents into the esophagus, which leads to tissue damage or esophagitis Treatment and symptoms are usually same only the level to which acid regurgitates is different.

You should visit a gastroenterologist to confirm the diagnosis and get a prescription.

GERD can be the reason for burning i my mouth and tongue and sour taste in my mouth and also stinging in my eyes in the morning or are those symptoms of a different disease?

They are all due to reflux of acid.
Other then stomach no part of our gut can tolerate acid,Due to reflux of acid into mouth food pipe etc the mucosal layer get inflammed and breeched causing buring sensation pain.Taste buds located on tongue are also destroyed hence taste sensations are altered.
