What Causes Abdominal Pain And Blood In Urine?

default
Posted on Tue, 30 Jun 2015
Question: Was in a recent car accident. I have abdominal pain. Went to the doctor and she did a urine test which showed blood. Renal panel was normal. She thinks the kidney was bruised in the accident. She said I could do physical therapy and light exercise in the pool. Also said to drink lots of fluids. This is contrary to what I have read stating that I should limit all activity for at least two weeks and until no more blood shows up in the urine. Also, should I actually be limiting my fluid intake to rest the kidney? Thanks.

Brief Answer:
No need to limit fluids and physical activity.

Detailed Answer:
Hello!

Thank you for the query.

Can you please give some more details about your symptoms? Where is the pain located? Do you have any nausea, vomiting, blood in stool? Is your abdomen bloated? Do you have fever? Have you had abdominal ultrasound or CT? If so can you please attach the reports?

Generally kidney injury is self limiting and self healing process. Even hardly smashed kidney (without active bleeding from arteries and veins outside the kidney) does not cause any serious problems. Limiting fluids is actually not a good idea. If there is any bleeding to ureter, drinking less fluids can cause blood accumulation and some obstruction. So it is rather necessary to drink plenty of liquids.

If its about physical activity, first few days are very important to limit it. But after this few days you should rather gradually get back to regular physical activity. If you have some mild injury of kidney, physical activity rather wont cause any damage.

Hope this will help. Feel free to ask further questions.
Regards.

No nausea or blood in stool. The pain is is on right and left sides of abdominal area. On the right side it is in lower right quadrant as well as right under rib cage. It is also under rib cage on left side. I have not had a CT done yet. I have a follow up with my primary care in ten days for another exam and urine sample. If there is still blood in the urine at that time she will order a CT? Do you feel a CT should be done sooner?

Are you a nephrologist?

Brief Answer:
CT should be done right after the accident.

Detailed Answer:
CT should be done right after the accident. No doubt in that. Please note that car accident injuries are very unpredictable. Sometimes what seems to be a mild injury turns into something serious. Spleen rupture for example does not give any symptoms at the beginning. Same is with kidneys.
So I suggest you to do not check if there is blood in the urine or not and have at least an ultrasound done.
