Brief Answer:
dysmenorrhoea

Detailed Answer:
Hello,
Thanks for the query to hcm,
The fact that you have had irregular heavy menses, and its the first episode itself, then considering that you are breast feeding but in the process of weaning , then the heavy menstruation is much more likely as prolactin levels tend to reduce and your ovulation restarts. If your menses have been resumed and you have been having unprotected sex then during fertile period, you may get pregnant, so you need to chalk your ovulation with an ovulation kit hence, else use protection.
First one or two cycles after lactational amenorrhoea are likely to be heavier and also associated with some clots and can be controlled with antifibrinolytics like TRANEXAMIC ACID tabs and pain can be controlled with MEFENAMIC ACID tab thrice a day.
It is still not evidence enough, being an isolated single episode that you may have endometriosis. However you may get a ultrasound pelvis done to rule out any ovarian cyst of adnexal endometriotic cyst if any. though before proceeding it is always better to rule out pregnancy with a urine pregnancy test to be on a safer side.
I hope i have answered your query,
Take NSAIDS like mefenamic acid , available on prescription , for pain relief and watch your next 2 cycles and maintain a record of symptoms for better evaluation.
Regards
