Question : Dr. Sheppe,
This a follow up to my inquiry from last week re: Duloxetine.
I have been taking the medication for a week taking it in the morning.
It makes me drowsy all day and wonder if I should be taking it at night. does it make a difference if I switch to taking it in the evening before bedtime?
In addition I am experiencing a loss of appetite as expected. This concern I have with acid reflux (taking a prescription of Prilosec) and being a pre-diabetic is making it impossible to put on weight. Besides the medication, is there alternative
care I should be seeking....in your opinion? If so, what specialty? Or is this all mental. I have been through every test the past 18 months with the exception of a neurologist. Is this another alternative course of action? Thanks for your kind assistance.

Brief Answer:
Private Consultation
Detailed Answer:
Hello, and thank you so much for using my direct private consultation.
It is perfectly safe and very reasonable to take the medication at night instead of in the morning. Sometimes people do find this medication to be sedating, and switching to evening is often helpful.
I do not believe you need any further workup or medical consultation. Your workup has been thorough and indeed too intensive... your issue is illness anxiety which will not be helped by more testing or consultation. Stick with weekly psychotherapy and the medication you are taking.
