What Causes Acidity And Frequent Stomach Infection?

Question: Hi doctor,



This is related to my stomach. I have been facing issues with stomach gas, poor digestion, acidity and frequent stomach infections.

I was having stomach bloating since about a month. On Sunday I ate out and since then I have stomach discomfort, slight nausea and urge to pass motion. Since morning there is stomach rumbling with headache and dizziness.

Please advise.

Thanks for query dear patient.
I am a Gastroenterologist and would be answering your query.

As per your query your over all digestion is poor.Digestion is the process in the alimentary canal by which food is broken up physically, as by the action of the teeth, and chemically, as by the action of enzymes, and converted into a substance suitable for absorption and assimilation into the body.
This digestion or catabolism is divided into two types –
1)the mechanical digestion of food that occurs in the mouth when it is physically broken up into smaller pieces.
2)chemical digestion that occurs in the gastrointestinal tract when the food is broken down into small molecules by digestive enzymes.This part is severely hampered in your case.

From swallowing to excretion every thing is a part of normal digestive process.

Now lets come to your problem one by one-


Acodity or dyspepsia is due to overproduction of acid sometimes due to the weakness of sphincter at gastro esophageal junction contents of stomach regurgitates back into esophagus causing severe irritation of mucosal layer of esophagus leadiing to heart burn

Suggestions
Please request your gastroenterologist to start you on PPI-antacids like Rabeprazole or Esomeprazole.Also, request him/her to add Domperidone 30mg or Levosulpiride(both are prokinetic)slow release once daily. This will slow down the reflux of acid back.Prokineic should be added in your regimen.
A combination of Rabeprazole with Domperidone would be the best.

An antacid containing local anesthetic (Mucaine gel or peptobismol) should be taken 2tsf thrice daily.(Oxethazaine +Aluminium hydroxide)) combination would be best for you take 2 tsf thrice daily with half cup of water.

Our gut consists of communities of good bacteria responsible for proper fermentation and food digestion. However, when these good bacterias are destroyed, food is partially fermented causing the release of gases. Take a probiotic like vizylac daily.The pathophysiology of gas and bloating flatulence is complicated. Understanding gut microflora, gas production, intestinal transit, intestinal propulsion of gas, and sensory function within the GI tract are essential for understanding symptom generation.The term gut microflora or microbiota means hundeds of species of bacteria which lives in our intestinal tractintestinal tract.These bacterias and yeast are responsible for proper digestion and fermentation of food. Hundereds of species of bacteria and yeast re found in colon of human intestine.In the same way thousands of these species florishes in our gut these bacteria play a vital role in digestuion and absorption of food.Even minor disturbances in gut microflora can lead to significant changes in gut function, including gas production.
There are various mechanism that can be related to increased bloating.

As told above If this microbiota is disturbed food is improperly fermented causing production of large amount of gases in gut leading to severe flatulence.Hence most commmon cause in most of the cases is disturbed microbiota.In some patient mobility of intestines is severly compromised leading to accumulation of large amount of gases in intestines.As persitaltic moments are week gas accumulates in intestines.Severe constipation or incomplete evacuation can also hinder the passage of gases.
-Small Intestinal Bacterial Overgrowth can be a reason of severe bloating .
Small intestinal bacterial overgrowth (SIBO) refers to a condition in which abnormally large numbers of bacteria are present in the small intestine, and the types of bacteria in the small intestine resemble more the bacteria of the colon than the small intestines causing improper and disturbed food digestion.


Medication suggested-(As you are in XXXXXXX you will get all of them without prescription)
Tab norflox TZ 1 tablet twice daily for 5 days.(Some time patient is suffering from chronis recurrrent gastroenteritis it would help eardicate bacterial or protozoal infection).
Cap Vizylac twice daily for 14 days(probiotic)
Tab Spamindon twice daily for 5 days (for spasmodic pain).
Syrup aristozyme 2tsf three times a day(to improve digestion).
Capsule rablet d once daily empty stomacg(gas acidityu and nausea)
Mucaine gel or peptobismol) should be taken 2tsf thrice daily.
Follow up after 7 days.

Test suggesteda are -
Stool routine and microscopy
Ultrasound of abdomen

General suggestion-
Drink plenty of fresh water daily.
Avoid juices they contain citric acid and will increase your acidity.
Drink two glasses of cold milk daily.Its alkaline and neutralise acid.
Take light easily digestible vegetarian food.
Avoid oily and spicy food.
No alcohol or smoking.
No aerated beverages or fast food like pizza etc.
