Question: I'm 82 years old. My PSA has been steady for many years at a safe level. It suddenly jumped up by eleven points between May 2016 and February 2017. Since it's a given that I have prostate cancer, I would like to know the various treatments recommended for my situation. The first thing that must be done, in my view, is to determine if the cancer is contained in the prostate, or has it spread? What is the procedure to determine if the cancer has spread. Thanks for query ,

Rising PSA steadily over a period of 6 months strongly susoected to be due to cacer of Prostate .

Since modality of treatment for cancer o prostate is entirely govered by the stage of the cancer it is primarily important to do following basic tests done to detremine the grade and stage of cancer .
1) MRI guided multi core biopsy of prostate ,This will elaborate whether cancer is confined to prostate gland or has spread to surrounding structure beyond capsule of prostate
2) CT Scan of abdomen to find out whether it has spread to lymph nodes
3) PET CT bone scan to rule out spread to bones

The further modality and line of treatment will depend upon results and findings of these tests .

The common modalities that are being practiced are
1) Radio Therapy
2) Chemical Castration
3) Hormonal treatment (Anti androgen )
either in combination or alone .

The treatment results are monitored by doing frequent levels of PSA and clinical presentation and modified according to levels of PSA .

I would suggest you to consult qualified Uro Oncologist or Medical Oncologist after you get the results of these tests .
