What Causes An Itchy Palate After Using Asthalin Inhaler?

Question: I get allergic rhinitis Nov-March every year. I take Montec-LC usually once in a day (at night mostly) almost daily. It give some relief though does not eliminate the problem. I also use Asthalin inhaler on need basis again usually once in a day during these months. If the problem is more I take inhaler 2 or 3 times a day. I get strong itching on pallet inside mouth some times specially at night which does not seem to have a cure. Also this results in blocked nose (usually left side) and snoring problem. In summers all these problems go away. Is it safe to Montec-LC for few months in a stretch (one per day)? What are likely side effects? Is there any other cure for itching on pallet and all these issues besides what I am doing ? Any other suggestion for the problem or a better solution?

I have read through your query and can certainly understand your concern.

The tendency to develop allergic, or IgE-mediated, reactions to extrinsic allergens (proteins capable of causing an allergic reaction) has a genetic component.

In susceptible individuals, exposure to certain foreign proteins leads to allergic sensitization, which is characterized by the production of specific IgE directed against these proteins. This specific IgE coats the surface of mast cells, which are present in the nasal mucosa. When the specific protein (eg, a specific pollen grain) is inhaled into the nose, it can bind to the IgE on the mast cells, leading to immediate and delayed release of a number of mediators.

So the line of treatment involves treating the release of mediators, as they are the causative organisms of your symptoms.

I would suggest you to do the following:
- if no laboratory tests where done, get a total serum Ig E and total eosinophil count count.

- Get room humidifiers, so as to keep your airways moist and will relieve the itching on the pallet

- take Montek AB which comprises of Acebrophylline and Montelukast twice daily for a period of 2 weeks

- I would also suggest you to talk to your doctor to change your Asthalin inhaler to an inhalational steroid which will give due relief.

- also wash and dry clean the curtains, bed roles so as to avoid any exposure to pollens.


Yes, you can take Montair LC for a few months but in some tolerance to the drug has been reported. I would suggest you to discuss with your doctor.

Other available suggestions include immunotherapy, which involves desensitizing to the allergens gradually thereby decreasing the allergic response.

Dear Dr Koshy,

I appreciate your detail answer. I have few thoughts regarding the same:
No Doctor last 4-5 years mentioned Montek AB, they either prescribed Montek LC or Allegra 120. They did ask me to take twice a day for 3 to 7 days Montek LC. with inhalers. Montek LC is 210mg tablet with different composition so I do have some concern that I am taking a 'strong' medicine for 2 weeks. I have started the same & feel better last 2 days. I do have a steroid inhaler which I use along with Asthalin sometimes. I use it only if I have stronger allergy which happens every year in Feb-March. As of now, I have / had pallet itching, left nose block, stuffy nose and snoring while sleeping. Most of these problems happen in evening-night at home, in office I am better or ok. But as I said I am feeling better with Montek AB now. Thanks.

Thanks for writing back,

I have read through your query and I'm pleased to know that your symptoms have subsided a little.

Montek AB is a new combination drug, like I said in my earlier answer, consisting of acebrophylline and montelukast. Montair LC contains montelukast and levocetrizine, which if taken for a while develops tolerance in most of the patients. Also Allegra contains fexofenadine which is an alternative to levocetrizine and has not proved much relief in patients with seasonal allergic rhinitis.

Ideally, Montek AB can be taken once to twice a day, since you feel that your symptoms have subsided after taking for 2 days, I would suggest you to continue for one week in the same regimen and then reduce it to once a day for another week. It is not advised to continue after 2 weeks. This is aimed to attack the mediators released in your body, thereby relieving the nasal congestion and other symptoms.

It is always advised to gargle mouth and rinse properly after the use of inhalers and if failed to do so, you will be prone to throat itching and hoarseness of voice. Also consider the use of lozenges which will soothen and give relief for your throat.

Meanwhile, I would suggest you to do the following for stuffy nose and snoring:

- use 2 pillows while sleeping

- use saline nasal drops and steam inhalation always before going to sleep

- may also consider the use of eucalyptus oil.
