Question : Hi i have had a itchy rash on my ribcage for about four months it has spread and marks are now on underside of breast. Skin has thinned i expect down to using hc cream. Skin is also discoloured and red patches

No i have not seen a doctor or dermstologist. The cream is just a standard over the counter ty

Skin lesions have to be seen

To diagnose a skin lesion, it has to be seen.
Please do not use any cream without doctor's advice as it will mask the original lesion.
As per your description, it looks like a fungal infection, Tinea corporis.
Consult a Dermatologist, get examined clinically and.get appropriate prescription.
