Hello and welcome,
The foreskin can get an overgrowth of yeast which can cause the symptoms you are describing. An antifungal/antiyeast cream such as Nizoral (ketoconazole) 2% cream, or miconazole, or clotrimazole may treat it.
I am hesitant for you to use an over the counter anti-yeast cream without your rash being diagnosed in person to determine if it is a yeast infection (rather than something else), and the extent of the swelling and constriction. You can try the anti-yeast cream I mentioned, but it doesn't work right away (it can take a few days). If your symptoms get worse in the meantime (more swelling or constriction), then do go in to see a doctor promptly. I say this because excessive swelling can prevent retraction and then make urination difficult, and also cause splitting of the foreskin and set it up for a bacterial infection.
