Question: I defecate into my underwear.From the after effects of radiation. What should I do?

Hi and thank you for asking!
I am Dr.Klerida Shehu gastroenterologist and I am happy to assist you.

You are reporting to have done radiotherapy and after that you have defecated into underwear( anal incontinence!!)
It is well known that radiotherapy has side effects like damage of rectal mucosa or damage of anal ring and anal muscles leading to anal incontinence.
I woul like to know more of you rmedical information befor egiving my recomandations :
1. do you have rectal bleeding?
2. do you have diarrhea/ constipation?
3. defecation into underwear is happening all the time or is occasional?
Otherwise at your situation I suggest as follows:
- try to eat helathy diet like fruits and vegetables reached in fibers
- start to do Kegel's execise(Contract the muscles of pelvic floor for a slow count of five and than release the muscles to a slow count of five.It will helpt o strength anal and pelvic floor muscles.
- continue to use Imodium it will help to tight anal sphincter.
If there is no improvemet after medical therapy than at cases such yours I recommend to use surgical therapy like injection of various materials to augment the function of the internal anal sphincter.
Injectable silicone has been shown to be effective in such cases.

Hi back
If you have rectal bleeding and diarrhea than you can consult with your physician to start:
- mesalazine and cortisone enema or suppositories.
If you continue to have anal incontinence after stopping rectal bleeding and diarrhea than you con use surgical therapy to tight anal sphincter.
