To recapitulate: Female/40 - knot of the size of a marble - tender - on butt cheek close to vagina and anus - not red - just a lump under the skin...
With the description you have given, this looks to be a small infected cyst or a boil.
But needs to be confirmed that it is not a fistula in ano tract.
Hence I would advise you the following:
- Clinical evaluation and local examination as well as per-rectal digital examination to see that it is not connected to the ano-rectal area or to confirm that this is a local sub-cutaneous infected cyst or so.
- To get proper prescription based antibiotic and anti-inflammatory medication to have the cure and surgery if required.
- The lumps in such a area should not be fondled or tried to pop up as the infection can really spread fast.
- You may apply liquid povidone iodine locally to keep the area sterile.
- Take laxatives if there is difficulty in passing motion or have hard stools.
