Question : I have hypertension and I get these anxiety feelings all the time mainly after a meal or a drink at night.
I also feel fatigue all the time.

1. I have gone through your reports. Your cholesterol level is higher than normal. however for further interpretation I would like to know few more details like If you are a diabetic too ? Do you smoke? Do you ever had stroke, heart attack or any other vascular disorder? What is your usual blood pressure level? All these will help to determine your risk and optimal therapy.
2. Your vitamin D & Vitamin B12 levels are low and need supplementation. This can be in oral form or injections. Do take a prescription from your physician.
3. if you go for a walk or daily exercise ? Do you get any symptoms at that time? If yes you need to see your internal medicine specialist or cardiologist.
4. Do continue with your blood pressure tablet.

1 I am not diabetic and I don't smoke. Never had heart attack but on 2-3 occasions had situation like anxiety, very acute headache and sweating. My usual blood pressure levels without medication is 140/95.
2 No medication for vitamins as of now.
3 I don't go for exercise or walk as I have disc problem and as already told you always fatigue like situation.
4 Is this medicine alright?

1. I calculated your 10 year risk for heart attack. 2 patients out of 100 will develop heart attack in next 10 years with a profile similar to yours. You are not a candidate for cholesterol lowering drugs (statins) as of now. However you must keep an optimal weight (body mass index of 23), exercise regularly (under supervision and after orthopedics consultation), avoid fried food and sweets.
2. Do consult your doctors for vitamin supplementation and for your disc problem. You should be able to exercise for a healthy heart.
3. Hipres (atenolol) 25 mg is a low dose beta-blocker. It is one of the recommended medicines for high BP, however I do not use it as first line drug now. I prefer ACE inhibitors or ARBs because they have better long term side effect profile than this drug. There is no urgency to change it; you can discuss with your physician at ease.
4. For your palpitation and anxiety feelings, it is difficult to access a correlation with heart disease if you do not exercise. the gold standard test to evaluate such symptoms is an exercise stress test. Do discuss with your cardiologist and orthopedician about it.
