As per your description, it looks like musculoskeletal pain.
As the pain is altered due to posture, it goes more in favour of musculoSkeletal cause.
I prescribe Skeletal muscle relaxant tablet containing Thiocolchicoside to my patients with similar complaints.
I also prefer to go for an Ultrasound abdomen to rule out any Kidney problem.
