Posted on Fri, 5 Dec 2014
Question: Hi, I am facing with back pain since last one year
Initially doctor asked me to get x-ray and couple of tests which then found normal. He prescribed calcium tablets and asked me to get the physiotherapy done. I followed this and got some relief.
Later the back pain start appearing again and this time doctor asked me to get the MRI scan done. The MRI scan also was normal, except they found cervical spondylitis. I have also done with vitamin D deficiency test which comes out normal.
Presently undergoing physiotherapy treatment since one week, Just want to understand whats wrong with my back....

Brief Answer:
DO NOT REQUIRE SURGERY,YOUR MRI LUMBAR SPINE IS NOT NORMAL

Detailed Answer:
Hi
Thanks for query,

You have to understand a few basic facts,
1.DEGENERATIVE DISC DISEASE ARE THE PRICE HUMAN RACE HAS TO PAY IN ORDER TO WALK ERECT.

2.THOUGH YOU DO NOT REQUIRE SURGEY,YOUR MRI LUMBAR SPINE IS NOT NORMAL. There is sacralisation of L5 predisposing to L45 PIVD and Schmorl nodes L3 signifying degeneration.
Do you know no schmorl nodes are found in aged quadrapeds because there spine is horizontal and supported on both side by legs(pillars). Due to erect posture blood vessels to disc get damaged early in life so any human by age of 30 has little or more disc degeneration

3. Now what you have to do
-Avoid prolonged sitting
-Sit with back supported only, Use proper chair ,do not use bike
-Do no bend forward, if needed kneel to bend
-Whlie standing too take support and do not keep legs straight
-Do back extension excercises daily
-may join swimming or brisk walk/run 40 min
-while lying down keep your legs bent , may use pillow there
-Do not lift heavy things

4. Sometime tension may cause such pain

Hope I was able to answer you query.

Thanks Doctor. Below are few follow-up queries I have:
- Physiotherapy treatment is going to help me?
- I do regular morning walk for 30-45 mins since last 3-4 years, stopped now to avoid any impact, should I continue?
- My blood sugar level found increased recently - fasting -146 and post lunch 160, no one in my family is diabetic. My Father (76) and mother (65) don't have any problem so far.
- Medicine can help in some way? I don't know swimming.
- I use bike to go nearby places.
- recently I was working 10-12 hrs, due to heavy workload. I am into IT industry and spent all the hours sitting in the office. Try to maintain posture due to backache.

Brief Answer:
I expected

Detailed Answer:
I expected you do a 10-12 hrs sitting job in IT sector ...stress on spine and mind..so you have the reason for your problems. Almost Every week I see one patient from IT sector with near normal MRI and back pain. Your diabetes too is attributed to stress.
Restart 40 min brisk walk/swimming(better learn it) , now you have another reason (diabetes).
Do not use bike. Do not lean forward while sitting. After 30-45 min get up or take rest.
And do not take mental stress. Strict 1400 Kcal diabetic diet and medicines if necessary. I think whole IT and management sector has to rethink , ultimately what they are going to achieve for whom, at what cost. Hope you understand now. Thank you.

Thanks Doctor.
So I believe I don't need to worry much and hope the pain would get cured by following your advice. You have not replied to my Physiotherapist query. Can you please elaborate more on 1400 Kcal diabetic diet? I was going to Gym earlier, which also I stopped due to back pain. Should I start again or it will add to the pain more. I was thinking to do back strengthening exercise but avoiding it to avoid more pain. Please suggest.

Brief Answer:
Consult diabetologist and good physiotherapist

Detailed Answer:
Diet chart you take from dietitian and consult diabetologist or physician.
Physiotherapist is very important. He will tell If you have muscle spasm now ultrasound therapy or TENS may help. Once this spasm subsides back strengthening exercises may be started. No gym. Thank u.
Note: For further queries, consult a joint and bone specialist, an Orthopaedic surgeon. Book a Call now.
