What Causes Bad Breath Despite Good Oral Hygiene?

Question: Hi, am 45 and noticed gum recession since I was in my 20's and gave it no importance. It is not from poor oral hygiene since I brush my teeth three times a day and floss regularly as well. I noticed I had really bad breath and a bad taste in my mouth. I noticed that when I applied pressure to my gum, pus would come out and it smelled foul. I went to the dentist and he told me I was brushing to hard and that it had wasted the gum and that a root canal would take care of the problem, he did the root canal and I did not have the problem for a couple of days but the problem returned after that. I went back to the dentist and he told me that the next solution would be to remove the tooth. Would that be the solution or is the infection in the gum. My gums look healthy and pink and I also noticed other recession on my other teeth. I have been dealing with this for the past 10 years what do you advise. Thank you.

This is Dr. Priyanka tiwari answering your query .

Read your query and understand your concern as you have undergone root canal treatment after that your problem returned again after couple of days see this same problem of pus discharge with bad breath can be due to infection either related to same Root canal treated tooth (formation of periapical abscess ) or infection in gums ( formation of gum abscess) .

I will suggest you to consult oral surgeon and get investigation (IOPA XRAY ) of tooth to check the extent of spread of infection and pus . If your xray reveals secondary infection in same Root canal treated tooth and tooth can be saved then treatment can be done is:
-Re-Rct ( Reopening of canal with proper irrigation of canal , maintaining aseptic canal preperation then allowing abscess to regress after that obturation is to be done ) .
If your xray reveals infection in gum (pus formation in gum ) then treatment can be done is Scaling and curretage .

Inmeantime I will suggest you to do Maintain proper oral hygiene by using soft bristle brush .
-use mouthwash chlorhexidine regularly .
- you can use waxed interdental floss to clean inbetween areas of tooth .
-Do warm saline rinses two - three times a day.
