What Causes Bad Breath?

Question: I am suffering from bad smell from my mouth. I visited doctor and she told all extrinsic deposits removed and tooth also got cleaned. Still I am facing the same problem. Then she told it's because of small deposits in gums. She asked to rinse using plax instead of normal water.

May I know what is the actual problem and perfect solution to get rid of the bad smell from mouth.
