Question : I have a bald itchy spot on my scrotum I would say it is about the size of a nickel

Detailed Answer:
Hi

You seem to have developed a patch of neuro dermatitis over the scrotal region.
It is basically a type of skin eczema which is quite common in men.
Apply a topical steroid cream like cortisone cream over the affected area twice daily.
Avoid rubbing or scratching the affected area since touching it tends to increase the eczema more.
Take some oral antihistamines like levocetrizine once daily at night to relieve the itching.
Wear cotton undergarments.
