Question: XXXXXXX i was diagnosed with IBS at the age of 12 with quite bad pains for a year and i was skipping school a lot. The pain started to ease down as i grew up. I am now 21 and have been having severe bloating and i cant let out gas at all. I find it so difficult to let out the gas. My stomach gurgles when i breath in it sounds so weird, feels like something is moving in there. I'm a very skinny person and over the past few months have lost a lot of weight. I've been having identical symtoms of H.Pylori. (stomach pain, pain after eating, pain relieved shortly after first meal, mental nausea) I went to my doctor 2 weeks ago where i asked for a H. Pylori test. It came back negative. I'm seriously confused right now as to what is going on. I'm guessing it maybe GERD. (GERD runs in the family) But i don't have that much pain in my upper stomach area. The pain feels located in the upper intestines/lower stomach possibly due to the gas. It's been causing me a lot of pain and anxiety/depression. Mornings are so difficult to deal with, i have to avoid eating at all after 6PM otherwise i'm guaranteed to have severe sickness and anxiety the morning. What do you think it could be? I have another appointment with my doctor on Tuesday but i don't even know where to look next.

Based on your symptoms, I am sure the problem is acid peptic related. Yes, the possibilities of GERD causing such symptoms are less, I suspect you to have duodenal ulcers or duodenal mucosal erosion due to hyperacidity. This is what leads to gurgling stomach.

You may need an endoscopy of the upper gastrointestinal tract, this helps not only to rule out GERD also helps to identify any acid related defect in the duodenum.

Anxiety and depression is one of the major and common cause for duodenal ulcers. Do discuss the same with your treating doctor and get managed accordingly.

Thank you for the reply!

I didn't realize your reply and I saw my doctor today who gave me Omeprazole 20MG. I will see how this goes for a week. Some days it's not too bad. The mornings are probably where i would say it affects my daily life.

I'm not sure what to say to your reply really, i'm quite shocked but happy to hear a direct answer from a doc for a change. One of my other problems tho is I think i can read things online and psychologically create these pains. Which is what i felt like i was doing for 2 weeks panicking i had H.Pylori.

What other reasons could there be for the symptoms i am describing? Is there another drug i should be taking to help heal the ulcers if they are there?

Unfortunately my doctor is only in on Tuesdays so i would need to wait until next week.

Thanks for writing back.

Duodenal stress ulcers or increased duodenal acidity leading to erosions fits to your description of the condition well. It is fine for you to try the drug you have been prescribed, also you can go for a better drug like Pantoprazole 40mg.

If there is no response, then it would be necessary for you to get the endoscopy done to rule out duodenal pathology.

Need not worry, the condition is not serious. The course with antacids like Omeprazole or pantoprazole cure the condition sometimes.
