Question: Hello doctor,
I am 23 male. Today evening when i went to the washroom, i noticed dried blood stain on my underwear making a patch on the backside of my underwear. I didnt have any pain nor i could find out where it came from..i dont know if it was dark red or bright red because it was dried up. Before this, when i went in the morning to the washroom there was nothing in my underwear. I am not having any kind of pain in penis nor my anus. I had sex last week for 3 continuous days. I played football today and so have overall bodily pain right now.
Past History: mild pericarditis, high BP..no other diseases

I am a smoker and i do take alcohol.

I am tensed now. Please help me out.

Brief Answer:
Possible exercise induced mild muscle break down.

Detailed Answer:
Hello, Sir.

I can understand your concern.

I have gone through your query in detail.

This single episode is not as alarming as you perceive it.

Sometimes heavy exercise like playing football can cause mild muscle breakdown and passage of blood pigments in urine. This episode in you could be all that.

I advise you to take 2-3 liters of water a day will prevent such symptoms in future.

Having said that, it is also important to evaluate further on a safer note in view of past history of nosebleeds, high blood pressure and pericarditis, the possibility of Gross hematuria has to be ruled out.

For this, I advise you to go for complete urine microscopic examination.

In case the report is fine, you can be rest assured.

If hematuria -either micro or macroscopic is detected, it needs further evaluation.

As of now, you need not be stressed much.

Post your further queries if any,
Thank you.

Brief Answer:
Possible internal hemorrhoids.

Detailed Answer:
Hello, Sir.

In such a case, the possibility of internal hemorrhoids is very high.

They can be painless as well.

They can rupture occasionally giving rise to bleeding per rectum.

For confirmation, you need to consult either a urologist or a GI surgeon for PER RECTAL examination and direct visualisation.

If these are detected either band ligation of sclerotherapy can offer a cure.

Post your further queries if any.
Thank you
