Question: Hi, I am XXXXXXX origin currently in UK. I was diagnosed with active Pulmonary TB in XXXXXXX 2016 and I was given standard four drugs treatment. All went fine for two months but suddenly I started coughing up blood with mucus. This did not happen before starting the treatment. I was feeling sore throat at that time and was confused whether it was from throat or lungs. The quantity of blood was not much. Only couple of drops but thick, similar to phlegm consistency. I was worried but doctor here said it can occur and is coming from lungs. Since then I was feeling sore throat on and off. Bleeding stopped after a week and did not occur for one month. Again it started two weeks ago and lasted one week. Again very small quantity. Now I feel more sore throat. Looking into mirror I can see it is red. One week before I noticed one white pus like small lump near right tonsil area. It remained for a week and now disappeared but I still have sore throat. I do not have breathing or swallowing difficulty. Actually throat feels normal when eating. I also noticed some wheezing sound when I take deep breath in sudden fashion. Occasionally I feel little discomfort in my ears. I am confused what this sore throat could be and why it has been for long time? Why ear pain on and off? My other doctor gave me two courses of Amoxicillin during last four months but no real improvement. I am worried. Please suggest what should I do and is this some serious thing? I have not lost weight. Please reply.

as you have said that you have been diagnosrd with tuberculosis and on ATT, blood in sputum is not that common..i think you should get a second opinion only after a CECT chest..
Superadded infections as well as bronchial carcinoma are frequently missed once a patient is diagnosed with Tuberculosis
If possible visit a pulmonary specialist

Hello doctor, Thanks for your prompt response.

I had gone through three chest x-rays so far and doctor did not notice anything abnormal apart from TB diagnosis.

I will ask them to refer me for CT. I am no smoker. Is this Bronchial Carcinoma possibility?

What about sore throat? Why it has been there for long time? Is it related to TB or something else?

blood in sputum should be promptly investigated..
Sore throat is rarely associated with so much bleeding, bleeding when cough most probably from lower tract..so better get investigated

hi doctor,

The blood in sputum has stopped now.

- What do you diagnose for sore throat?
- Could it be some bacterial infection?
- Why amoxicillin has not worked on two occasions?
- Why sometimes discomfort/pain in ears?
- what was that white pus filled like lump near tonsils? It has disappeared now but what was that? I do not have swollen tonsils at the moment. I think my uvula is little enlarged and touching back of tongue.
- I sometime have reflux kind of symptoms such as heartburn, something stuck in throat etc. But not regularly. My sore throat is for long time.
- Should I be worried that this could be serious thing such as cancer??

Hi
its good that bleeding has stopped..it might be from the inflammation of pharynx..the white spot on tonsil was probably a pus point due to tonsillitisand as the infection has cleared, the pus has gone..uvular inflammation will also be resolved in few days..
Amoxicillin has worked on the infection,that's why infection is so better now
for reflux you take some antiacid, avoid spicy foods, alcohol and tobacco..
there doesn't seem to be anything serious..just be compliant with the medicines
