What Causes Blood In Stool, Inflammation And Infection?

Question: Hi,
Couple of months back, I observed blood in my stool. when I consulted a doctor, he said there was inflammation and an infection but not piles. He prescribed Tinidazole 500, trenaxa, aceclofenac and pilorute(troxerutin zinc, calcium dobesilate, phenylephine, lignocaine with hydrocortisone cream). Also advised me be on a fruit and liquid diet. The medicines worked and I was relieved after 5 days and I discontinued the course(7days). Now it is back. Also there infection(redness) on the inner thighs(not sure if it is the same) and there is itchingness as well. Please help.

Thanks for the query.

Your description suggests inflammation & infection at anal area with fresh blood in your stools.
Looking at your age, the possible causes are- bleeding piles/ anal fissure. Your doctor has already excluded piles ( Still it needs to be excluded by rectoscopy- specially if the bleeding is more),- so it's mostly anal fissure.

Anal fissure is a condition where anus is broken or torn due to bad local hygiene, chronic constipation, Intestinal problems like crohn's disease/ ulcerative colitis/ chronic amoebiasis. There is inflammation & some times infection of fissure- leading to spasm of anal sphincter, painful motions & bleeding in stools which usually stains the periphery of stools. Itching follows.

This condition needs a prolonged treatment & care of the lesion, change in life style. When this is not followed- recurrence is common.

Management consists of-
1 - Acute phase of infection as per the severity of symptoms with metrogyl, suitable antibiotic, anti inflammatory drugs, lacto- bacillus preparation to normalize rectal flora.
- stool softener/ treatment of loose motions., schist's bath
2- Maintenance-It consists of-
- Investigation & treatment of the basic cause in intestines with help of gastroenterologist.
- Avoiding straining for stools, constipation. Take stool softeners. treat loose motions promptly.
- Increasing dietary fibers & water intake./ daily exercises
- Maintaining anal cleanliness after deification.
- Schitz's bath twice a day with application if ointment( Antrolin/rective) for healing.
- Have an ideal/regular life style with positive approach

The prolonged care with maintenance of good bowel movements along with treatment of bowel problem, mostly solves the problem.
Surgery is needed in non responding patients. It is done by rectal dilation or Lateral sphincterotomy.

Regarding the itching and infection of inner thighs, please describe in detail. If possible please upload a photo of the lesions.

I have had constipation frequently all my life. I eat outside. 5 months ago I was diagnosed with duodenitis. Was prescribed sompraz D 40 for 2 month(2 tabs for 1 month and 1 tab for one month). I use to have burning sensation if I skipped a meal or change my timings. Since then, I eat every meal on time. The is no burning sensation now except if I have a light meal or delay or skip a meal.
Few doubts:
You said: Avoiding straining for stools, constipation. Take stool softeners. treat loose motions promptly.
When I take a lot of water, part of the faeces is solid and part of it becomes watery. Is this ok or does it suggest indigestion? ( I have fruit juice for breakfast and lunch and rice with curry for dinner)
What are stool softeners? Are they some kind of foods or medicines?
You said: Maintaining anal cleanliness after deification.
I clean with plain water and wipe with a towel. Do I need to use soap? If so please suggest a good soap. Anything else I need to do?
What is schist's bath ?

They are small irregularly shaped mildly red bubbles. There is dryness as well. i have also observed a small lump in testicle. And on the upper skin of the foreskin, I have observed peels of dry skin come like dandruff.

Thanks for the follow up query.
I am impressed with your success to manage your Duodenitis problem by changing your eating habits.I am sure this attitude of yours will help you in overcomming your present problem too!
* By avoiding straining ( constipation) for stools I mean-
- To change your eating habit( more water intake,as you are already have )along with rise in fiber intake helps in avoiding constipation.( some un absorb-able fibers soak water and thus make stools soft/increase bulk of stools , thus helping normal bowel movements)
- Regular exercises ( specially for abdominal muscles),walking
- having optimum sound sleep/avoid tension.
though you are eating out- you can try to increase fibers in diet by eating fruits ( with skin whenever possible) instead of juice. Add salads, green vegetables XXXXXXX preparations,baked potatos,steamed sprouts in your diet.
* Loose motions are more than three watery/painful stools /day
I don't thinkyou have loose motions.That is also not a sign of indigestion. Adding more fibers ti diet will improve stool form.
*Stool softners are the preparations that make stools soft by rising it's water content. They differ from laxatives by not causing intestinal stimulation/spasms
(Dulcolax stool softner/Lactulose )
* About anal cleanliness I would suggest use of tissue papers in place of towel (to avoid reuse/ friction)
* Schitz's bath- It is a special way to foment anal part- by sitting in warm water ( in a round tub) Making the area clean/ fomenting it -thus relieving pain/inflammation & encouraging healing. done twice a day followed by ointment application.
* I am sorry, you could not upload the image,still from your description-I feel you have a fungal infection affecting your inner thighs,testicular,penile area. This condition c an be treated by anti fungal agents, antibiotics if required.
Similar condition can be seen in exaema/allergic dermatitis.
I would advise for consultation of a skin specialist for exact diagnosis & specific treatment.

I met a skin specialist. He diagnosed it to be fungus. Prescribed a cream which worked great. No itching after first application. The redness and bubbles are slowing subsiding.
